Job Summary: 

The certified diabetes educator plans and conducts educational programs to assist in the management of type 2 diabetes, type 1 diabetes and pre-diabetes. The educator meets with patients either one-on-one or in a group setting to provide education and medical nutrition therapy services. Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods/models. 

Duties and Responsibilities:

  • Provide educational services to adult and geriatric patient populations
  • Coordinate patient care within an interdisciplinary team including endocrinologists and nurse practitioner along with other support staff.
  • Assess and identify risk factors
  • Develop care plan and set goals with patients
  • Monitor and communicate outcomes
  • Manage the accreditation through the American Diabetes Association by tracking patient data and completely annual assessments.
  • Provide training on insulin pump therapy along with continued support and management for patients.
  • Help navigate process of ordering insulin pump and continuous glucose monitoring supplies. 
  • Offer support and education to medical staff on diabetes management. 

Required Skills and Abilities:  

  • Prepare, review, and maintain current instructional materials on diabetes
  • Provide technical guidance and direction in development of diabetes programs and protocols
  • Teach from current and approved Education Curriculums
  • Assist with development of policies, procedures, and programs
  • Provides one-on-one and group teaching of diabetes management and insulin pump therapy to patients
  • Collects DSME program participant assessment data, develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME program participants.

Education and Experience

Bachelor's and/or Master's Degree in Nursing, Nutrition, Education, Dietetics, Science, Graduate, Management, School of Nursing, Healthcare, Food and Nutrition
