GENERAL SUMMARY
The role of the CCMA (Clinical Support Operations Department) involves providing lay coaching to Jefferson County residents 18 years of age or older related to health care and chronic disease management. Working with Cooper Green patients, the CCMA-Clinical Support will entail facilitating appointments with the patient educator, the dietician, social work, clinical pharmacy, primary care or other needed service and screenings. Additionally, the incumbent will partner with the outreach coordinator, on networking and relationship building opportunities with key businesses, agencies, and healthcare organizations within Jefferson County, AL and the City of Birmingham, AL. Further, this role is responsible for engaging with prospective patients, introducing Cooper Green Mercy Health Services at community events, and educating the community on the services offered at the facility. The CCMA-Clinical Support will represent Cooper Green at public events such as health fairs (at both indoor or outdoor venues; may be scheduled during the week or on the weekend) in the community and does not shy away from public speaking, connecting with and influencing others, or negotiating.
KEY RESPONSIBILITIES
• Helps patients understand medical advice and recommendations.
• Work with Population Health Worker communicating, implementing, and executing population-level initiatives to improve the quality, efficiency, and cost of healthcare services.
• Completes and submits reports, based on work done in the community, as requested by either entity
• Provides names and contact information of community members who would benefit from Cooper Green enrollment; reaches out to assist with enrollment proactively or when needed (e.g. when literacy is identified as a barrier, when assistance is needed at the person’s home in collecting documents, etc.).
• Represent Cooper Green at public events such as health fairs (at both indoor or outdoor venues; may be scheduled during the week or on the weekend) in the community.
• Assists with quality improvement and research projects as needed; identifies obstacles to improvement. Identifies and communicates opportunities for process improvement.
• Other duties as assigned.
Position Requirements:
MINIMUM QUALIFICATIONS
Required:
• High School Diploma or Equivalent
• Current, valid CCMA certification from NHA or AAMA.
• At least one year CCMA experience. Work experience may NOT substitute for education requirement.
• Must possess basic knowledge of computers, PC applications, software, and database management.
• Current CPR/BLS certification via the American Heart Association or American Red Cross is required.
• Access to reliable personal transportation with a valid driver's license and auto insurance required
• Auto insurance must include: $100,000 of bodily injury per person; $300,000 per accident for bodily injury liability; and $100,000 per accident for property damage liability.
WORK ENVIRONMENT
• This role may require lifting up to 50 lbs.
• The work location is onsite (including occasional travel throughout Jefferson County, AL & Birmingham City, AL).
