POSITION SUMMARY:

Seeking a dedicated and compassionate Patient Navigator for our LGBTQ+ Healthcare Center with strong cultural competency and a commitment to serving LGBTQ+ communities. This role requires sensitivity, respect, and an understanding of the unique experiences of LGBTQ+ individuals. LGBTQ+ community members and allies are strongly encouraged to apply.

The Patient navigator works with Stony Brook Medicine Gender Affirming providers to ensure that the full continuum of care needs of our LGBTQ+ patients are identified and that appropriate services are accessed, coordinated, and monitored.

The selected candidate should understand that weekend and holiday hours may be required due to the needs of the organization. 

 

RESPONSIBILITIES:

  • Meets with and screens patients for program eligibility and overall needs, to direct patients internally and externally to needed services.
  • Coordinates, promotes, and ensures linkages to medical services. 
  • Conducts intake and reassessments to identify psychosocial needs and risk factors, care coordination needs and barriers to health care. 
  • Develops, implements, and updates individualized care plans in partnership with patient and medical team.
  • Serves as a healthcare system navigator to ensure that patients can access crucial aspects of health care. Assisting with linking to care services, health insurance, transportation, etc.
  • Provides health education, specifically sexual health and risk reduction education, to patients to support their full engagement in care.
  • Participates in case conferences with providers to ensure service coordination and continuity of care.
  • Participates in interdisciplinary staff meetings. 
  • Participates in continuous quality management program. 
  • Other duties as assigned. 

REQUIRED EXPERIENCE AND QUALIFICATIONS:

  • Bachelor Degree or higher in health related field (i.e. community health, Social Work). 
  • Previous experience working with LGBTQ* Community. 
  • Ability to handle multiple tasks and responsibilities. 
  • Strong interpersonal skills in order to interact in sensitive and/or complex situations. 
  • Ability to work independently as well as part of a team. 
  • Culturally and linguistically competent service delivery. 
  • Proficient computer skills including Microsoft Office applications such as EXCEL. 

PREFERRED EXPERIENCE AND QUALIFICATIONS:

  • Bilingual in English/Spanish. 
  • Knowledge of HIV and HIV PrEP. 
  • Knowledge of support services in Suffolk County. 

As part of employment at Southampton Hospital, there may be occasions when travel to other locations is needed to support operational needs.

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established based on relevant experience.

The above salary range (or hiring range) represents SBSH’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

Prior to start date, the selected candidate must meet the following requirements: 

Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Southampton Hospital’s Employee Health Services*

  • Complete electronic reference check with a minimum of three (3) professional references.
  • Successfully complete a 4 panel drug screen*
  • Meet Regulatory Requirements for pre-employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).
