As a Business Development/Clinical Liaison, this position is responsible for census development and coordination of new and current resident admissions and transitions within the continuum of care, to include admissions into the skilled nursing facilities on both campuses.  The incumbent will work closely with internal and external entities, both licensed and non-licensed health and wellness teams, to identify individual resident care needs and facilitate transitions to the appropriate level of care. As a professional member of the health care operations, this position is expected to work closely with the health services leadership team  and assist in continuously scanning both internal and external environments, identify opportunities to capitalize on competitive advantages, participate in performance improvement activities and promote our services within the local medical community. The ability to develop various reports, together with track, trend and analyze data, and proficiency in developing and executing action plans. This position is required to perform on-call responsibilities.

 

  

 

  

 

Work Schedule: Core work hours are from 8:00 am to 5:00 pm, Monday thru Friday; however, special projects or assignments may require working additional hours to include evenings, weekends, and holidays.  This position is on-call for admissions inquiries every other weekend.

 

Work Location: Primary office is located at The Mission; however, same day travel within the greater San Antonio area is an essential part of this job.  When this occurs, mileage reimbursement is authorized.

 

 

What you will do in this role:

 

  1. Coordinates admissions and ensures proper DME equipment is available.
  2. Ensure timely turnaround of referrals to maintain a good relationship with hospitals.
  3. Conduct weekly Visits to hospitals, rehabs, ARU and build rapport with case managers.  Maintain a list of all case managers in community.
  4. Conduct follow up visits with specific case managers who have referred to facility.
  5. Knowledge of PCC and input admission information.
  6. Notify IDT of MOT on all admissions.
  7. Visit all CCRC residents in the hospitals and provide update to IDT Via hospitalization list.  Conducts in person visits as needed to potential residents.
  8. Assists accurate and timely completion of admission agreements and collects all necessary forms and documents, verifies insurance coverage and Medicare coverage, and identification of responsible party information required for admission of resident.
  9. Assures discharges and transitions are routed to appropriate level of care to ensure appropriate discharge planning. 
  10. Responds to all inquiries and interviews of potential residents, family members and discharge planners to collect accurate data on admissions inquiries.
  11. Planning and coordinating campus tours, information sessions, set up quarterly events at least quarterly.  Assist in tours for potential referrals sources and inform administrative staff when not present in facility.
  12. Attend daily stand up or daily stand down meetings as able.
  13. Work closely with facility IDT to being in Admissions and ensure successful stay.
  14. Partners with the Marketing staff and acts as liaison between transferring case manager or hospital and community; seeks ways to further enhance the relationship between Blue Skies of Texas and the at-large community.
  15. Assist the BOM in execution of Common Working File for applicable Medicare clients.
  16. Send out a fax blast each week identifying available beds.
  17. Maintains statistical data concerning SNF admissions and trends, analyses, along with development and execution of strategic plans to improve outcomes.
  18. Be on call every other weekend for potential referrals.
  19. Attends corporate, facility and departmental meetings, in-service sessions, seminars, and other meetings, as required.
  20. Participates in performance improvement activities.
  21. Performs other duties as assigned by Supervisor. 

 



Qualifications

Certifications/Qualifications: 

 

  • Direct experience with skilled nursing community admissions and demonstrate comprehension of Medicare, General Knowledge of PDPM, Managed Care and private insurance requirements for admission to a skilled nursing community.  
  • Required: LVN or RN in good standing with, the Texas Board of Nurse Examiners, at least three years of direct experience with skilled nursing community admissions
  • Required: Working knowledge of basic office equipment and machinery required along with intermediate knowledge and skills in working with the Microsoft Office suite of products including Outlook, Word, Excel, PowerPoint, Access, and Publisher.  
  • Required: Must possess a current, valid Texas driver’s license with good driving record, remain insurable by Village carriers, and apply for and obtain a military base pass within six weeks of employment.  
  • Preferred: Familiarity with health care computer systems applied to such operations. Training will be provided to familiarize incumbent with work-related equipment to ensure safety and proper operation during use.

 

 

Internal Control: Within the scope of position duties, responsible for seeing that operations are effective and efficient, assets are safeguarded, and applicable laws, regulations, policies and procedures are complied with.  This position will handle confidential information and must treat such with appropriate sensitivity.
