JOB SUMMARY
The Bilingual Patient Navigator serves as a trusted advocate, guide, and support resource for patients receiving care at the Federally Qualified Health Center (FQHC). This role helps patients and their families overcome barriers to accessing timely, affordable, and comprehensive healthcare services. The Patient Navigator provides education, resource connection, follow-up, and coordination of care—ensuring patients fully understand their care plan in their preferred language and can successfully navigate the healthcare system.
ESSENTIAL FUNCTIONS
• Serve as a primary point of contact for patients needing assistance navigating medical, dental, behavioral health, and specialty care services.
• Provide interpretation and translation services (English/Spanish or other needed language) to support patients in understanding medical instructions, insurance, and care plans.
• Assist patients with scheduling appointments, referrals, transportation, and follow-up care.
• Educate patients on available health center programs, community resources, and financial assistance options.
• Support patients in completing applications for Medicaid, Marketplace insurance, sliding fee programs, and other benefits.
• Work collaboratively with clinical teams, front desk staff, social workers, and community partners to improve patient access and reduce care gaps.
• Track and document patient encounters, referrals, and follow-up efforts in the Electronic Health Record (EHR) and care management systems.
• Conduct outreach calls to remind patients of appointments, preventive screenings, and chronic disease management visits.
• Identify social determinants of health needs (housing, food insecurity, employment, childcare, etc.) and provide appropriate referrals.
• Promote health equity by advocating for patients and reducing cultural and language barriers in care delivery.
Schedule:
Monday – Friday, the hours of operation, 8:00 am- 5:00 pm, with potential extended hours.
EDUCATION AND EXPERIENCE
• High school diploma or GED required, associate or bachelor’s degree in social work, Public Health, Human Services, or related field preferred.
• Bilingual proficiency in English and [Spanish/other applicable language] required (written and verbal).
PHYSICAL DEMAND
• Frequently sitting, standing, and walking within the clinic.
• Occasional lifting of up to 20 pounds.
• Extended screen time for documentation and outreach
KNOWLEDGE, SKILL, ABILITIES, AND OTHER CHARACTERISTICS
• Experience working in a healthcare, community health, or social services setting preferred.
• Knowledge of community resources, public health programs, and patient support services.
• Excellent communication, problem-solving, and interpersonal skills.
• Ability to maintain professionalism, empathy, and confidentiality when working with diverse populations.
• Proficient in Microsoft Office Suite, Electronic Health Records (EHR), and patient scheduling systems.
