Graham Windham's serves children with complex, significant mental health needs in their homes and communities, thereby decreasing the need for placement to intermediate psychiatric inpatient and residential treatment facility levels of care. The goal of the HCBS Waiver program is to give families and youth the supports that are needed in order for the youth to remain at home with their families and in their community.
About the Position
The position is primarily responsible for rendering services that are outlined on Child’s Care Plan to children and youth who have developmental, medical or other health needs.
Duties & Responsibilities
o Ensures the timely and quality execution of services outlined in each child’s Care Plan.
o Collaborates with stakeholders including the Care Coordinator/Supervisor (who manage service planning for program enrollees) to advance the achievement of
outlined service goals.
o Documents delivered services as required by the program (i.e. Progress Notes and other forms outlined by the program.)
Minimum Qualification
Community Self Advocacy Training and Support
o Bachelor's degree
o Experience working with children with related experience
Respite: High School Diploma or General Equivalency Diploma (GED)
Caregiver/Family Supports and Services:
o High School Diploma or General Equivalency Diploma (GED)
o Experience working with children and youth
