Position Summary: 

The Behavioral Health Patient Navigator operates as a healthcare professional who guides patients through the behavioral healthcare process and overcomes obstacles to wellness for our BH patients. The BH Patient Navigator will join a dedicated multidisciplinary team to provide comprehensive support services from the Behavioral Health Department to patients across the lifespan. The Behavioral Health Patient Navigator will play a critical role in the coordination of internal and external BH services, including effective communication with patients from diverse backgrounds, internal teams, external stakeholders, and the community. 


Program Specific Essential Duties and Responsibilities:

    Provides culturally sensitive, strength-based case management support and supports care coordination for clients of all ages, creating a welcoming and understanding environment for diverse backgrounds, utilizing interpretation services when indicated.

    Works directly with FHCW’s team of BH Providers to increase access to support services and engagement through identifying needs and connecting patients to resources to address mental health, medical, and SDOH needs.

    Assists with referrals from internal BH Team and tracking management activities, including but not limited to referrals to partial hospitalizations, CBHI services, external outpatient therapy, community psychiatry, neuropsychic testing, etc. . 

    Maintains a list of all active referrals and ensures patients are not lost to follow-up if at all avoidable.

    Communicates with referring providers at FHCW to keep them updated on the referrals process.

    Assists patients with completion of medical and BH forms and applications including but not limited to DMH applications, DDS applications, releases of information, etc.

    Assist with recall list for patients who missed scheduled BH appointments or screened positive for a BH issue.    

    Cultivates community-based partnerships with local and surrounding agencies to exchange information and resources and provide referrals to needed services in the community.

    Explore and address barriers to treatment with high-risk patients to ensure adherence to health (Medical/Behavioral Health) care goals.    

    Complete and document social determinant of health assessments for patients enrolled in BH as needed.  

    Report to BH Team and document progress towards meeting the needs identified in the social determinants of health assessment.  

    Prioritize and provide on-demand support as well as manage scheduled appointments to individuals, couples, and/or family adhering to the norm of a fast-paced primary care environment

    Maintain records and documentation of all patient contact activity and client data in the EHR in an accurate and timely manner.

    Participate in weekly clinical supervision, and seek peer consultation as needed

    Continually evaluates service and maintains a productivity level that adequately meets the goals of the department and ensures access to the clients seeking services.

    Any and all other duties as assigned by BH Director or CCO.


Qualifications and Education Requirements:

    Excellent knowledge of available community and resources

    Demonstrate effective follow through with all tasks and referrals

    Shows therapeutic communication and interpersonal skills


One of the following combinations of education and employment experience must be met in order to be considered for the position:

Bachelor’s degree or above in social work, psychology, or related human services field.           One-year experience in patient care and care coordination experience preferred.  Bilingual/bicultural in Spanish, Haitian Creole, Arabic or Portuguese, preferred.
