At Esperanza, we recognize that behavioral health is key to overall wellness. Our Behavioral Health Consultants work in close collaboration with medical providers to address mental health needs in the context of primary care. This role involves delivering individual and group interventions, brief therapy, and crisis response, helping patients of all ages reach their health and life goals through accessible, holistic care.
Deliver culturally sensitive, strengths-based, trauma-informed care
Provide brief clinical interventions to patients during medical visits
Address mental health concerns including depression, anxiety, trauma, grief, developmental needs, and substance use
Collaborate with medical providers on treatment planning and shared care goals
Develop and facilitate innovative group counseling formats
Offer crisis management and safety planning
Document services and outcomes in the Electronic Medical Record
Refer patients to higher levels of care and connect them to community resources
Fixed MondayβFriday schedule (no on-call, no evenings, no weekends)
