Qualifications

JOB SUMMARY


Under the direction of the Chief Community Impact Officer, implement behavioral health treatment, brief interventions and 
solutions-focused direct diagnostic and therapeutic services to individuals and families. Provide behavioral health 
counseling as a member of a multidisciplinary group of healthcare delivery professionals providing care in a coordinated 
fashion and empowered to work at the top of their professional training.


SPECIFIC JOB DUTIES


1. Works with individuals, families, and groups to address and treat emotional and mental illnesses and to promote 
wellness.
2. Works closely with members of Team-Based care unit to maximize the level of care.
3. Performs intake histories on patients seeking addiction treatment and assists with clinician visit planning.
4. Collaborates with clinicians providing treatment for addiction. Assists with patient follow-up and referral to 
external partners for addiction treatment when appropriate.
5. Provides diagnostic and therapeutic services in accordance with general standards applicable to mental health 
professionals.
6. Provides individual and/or group counseling services to children, adolescents, adults, elderly, and families. 
7. Adheres to and upholds the ethical standards and guidelines of the profession.
8. Documents work in accordance with Union Community Care standards in the areas of diagnostic evaluations, 
progress notes, treatment plans, and discharge procedures.
9. Participates in education and other activities necessary to maintain competency in clinical work.
10. Actively participates in clinical team and staff meetings.
11. Performs other work-related duties as assigned.


POSITION REQUIREMENTS


A current clinical license to practice as issued by the Pennsylvania State Board of Social Workers, Marriage 
and Family Therapists and Professional Counselors or, a current license to practice Psychology by the Pennsylvania State Board of 
Psychology.


Excellent communication (verbal and written) skills.

Ability to deal effectively with a variety of people and work in a team environment.

Ability to multi-task, prioritize and work under deadlines.

English proficiency required

ORGANIZATIONAL INVOLVEMENT

This position is required to participate in mandatory all staff meetings, team meetings and trainings.
