This position is responsible for planning, arranging, providing, and monitoring services for consumers enrolled in the Comprehensive Community Services (CCS), Coordinated Services Team (CST), and Children’s Long-Term Support (CLTS) programs. The role is supervised by the Program Supervisor within the Division of Community Programs.
Conduct initial and annual clinical assessments
Develop and implement person-centered service and crisis plans
Ensure services are delivered in a culturally, linguistically, and developmentally appropriate manner
Provide psychoeducation, skill development, and outreach to youth and families
Facilitate service referrals and monitor progress toward independence
Engage natural supports in recovery-focused planning
Coordinate with healthcare providers and social service agencies
Support families in transitioning youth to adult services
Maintain timely and accurate documentation per agency and state guidelines
Participate in staff meetings, supervision, and training
Provide coverage for department staff as needed
Bachelor’s degree in behavioral science and 1,000 hours of supervised post-degree experience working with individuals with severe mental illness
OR
Bachelor’s degree in another field and 2,000 hours of supervised post-degree experience with the same population
Knowledge or experience with trauma-informed principles (preferred)
Valid driver’s license, access to a personal vehicle, and auto insurance
Ability to communicate effectively, manage documentation, and work with diverse individuals and families
Familiarity with state/federal service regulations and community resources
Capacity to work both independently and as part of a multidisciplinary team
Must be proficient in use of computers and standard office software
Work performed in home, community, and office settings
Must be able to lift up to 20 pounds
Frequent travel within and occasionally outside of the county
Requires 8-hour workdays, 40-hour workweeks
May occasionally interact with individuals in crisis or high-stress situations
