ESSENTIAL JOB RESPONSIBILITIES:

The Case Manager will work along with the ESL team to provide services aimed at reducing or eliminating

homelessness for individuals with serious mental illness or co-occurring serious mental illness and substance use

disorders, who are experiencing homelessness or are at imminent risk of becoming homeless. The CM will provide an

array of services including street outreach, case management and services not supported by mainstream mental health

programs.

Services under this position and program will include assistance to these participants:

? Adults who are eighteen (18) years of age and older with a serious and persistent mental illness and/or

co-occurring disorder

? Families with children who have emotional/behavioral disorders

? Individuals who are chronically or homeless as defined by HUD and/or at risk of becoming homeless

(definitions are available)

? Individuals must be in Ouachita Parish

Locate individuals who otherwise would not be served

? Engage in a trusting relationship with these individuals (program participants)

? Screen for housing and other service needs of the participant

? Assess health and behavioral health needs of the participant

? Develop Individual Service Plans (ISP), training will be provided

? Provide direct care and services as able

? Utilize the Northeast Delta Human Resources Authority (NEDHA) form to make referrals to service providers

in the integrated care system and outside the system

? Refer participants to housing, health, and behavioral health treatment resources

? Attend and actively participate in regularly scheduled staff meetings and as assigned by supervisor

? Enter data into the Homeless Management Information System (HMIS), training will be provided

? Complete the Outreach Data form on all individuals contacted, including persons who receive outreach only and

those engaged or re-engaged in treatment or other social services

Arrange occasional transportation of participants for service, or if required, transport participants in your own

reliable vehicle

? Assist in the client exit process on the following basis, the participant:

  • Meets maximum program time allotment
  • Obtains permanent housing
  • Transitions into mainstream resources
  • The participant chooses to leave the program (planned or unplanned)
  • Has 90 days of no contact with the program or the OS

? Provide aftercare case management to ensure the client is stable and transitioned smoothly

? Attend and actively participate in required training and training as assigned by supervisor

? Demonstrate the ability to identify with project and Easterseals objectives; adequately, constructively and

directly communicate these objectives.

?Outreach is defined as face-to-face interaction with people experiencing homelessness. Outreach shall take

place on the streets, in camps, under bridges, abandoned buildings, in temporary motels, shelters, meal sites,

and wherever future participants might be located

An individual may be enrolled in the PATH program when the eligible individual and PATH provider have

mutually and formally agreed to engage in services



Requirements

QUALIFICATIONS:

  • Bachelor’s Degree in Social Service or Human Services related field is preferred but not required
  • Must have own reliable transportation and operator’s insurance
  • Willing to travel to the locations where future participants may be found
  • Able to multi-task and meet deadlines.
  • Excellent written and verbal communication skills.
  • Proficiency in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate. ?
  • Must be a team-player

PHYSICAL REQUIREMENTS with or without reasonable accommodations:

  • The employee is regularly required to operate a computer, file and retrieve written documents, and communicate with others on the phone and in person.
  • The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items.
  • Ability to move independently within the facility and community.
  • Must be able to travel and meet with participants

WORKING CONDITIONS:

  • Work is performed in a normal working office setting that is environmentally controlled and out in the field.
