Job Description

The Behavioral Health Case Manager – Domestic Violence Prevention and Education (BHCS-DVPE) serves as a multi-disciplinary patient care team member for NATIVE HEALTH’S Behavioral Health Department. The BHCS-DVPE provides case management services that support domestic violence intervention, prevention, and education services, and coordinates care to internal and external supportive services. The BHCM-DVPE represents the company professionally and positively to enhance and promote the core values and mission of NATIVE HEALTH (NH), consistently exercising utmost discretion, diplomacy, and tact in customer transactions.

KEY ACCOUNTABILITIES:

  1. Provides, coordinates, and facilitates non-intensive case management services in support of Behavioral Health patients and providers, as related to domestic violence (DV) clients and other general case management services of BHD.
  2. Participates in community engagement and delivers group services and education about domestic violence and sexual assault in multiple settings.
  3. Participate in quality improvement and quality assurance activities for the department.
  4. Provides support to the organization as needed.

KNOWLEDGE, SKILLS, AND ABILITIES: The following factors are essential to this position, and applicants must provide evidence that they possess these required factors to qualify minimally.

 

  1. Proficient knowledge of case management practices.
  2. Proficient knowledge of domestic violence and sexual assault concepts, theories, and best practices.
  3. Knowledge of community resources, including existing social and behavioral health resources and referral agencies available to patients and victims.
  4. Knowledge of outreach protocols and safety measures.
  5. Knowledge of program planning concepts and practices.
  6. Knowledge in health education concepts and practices, specifically in domestic violence and sexual assault.
  7. Ability to establish rapport with participants, gaining their trust, and engaging them.
  8. Proficient in motivational interviewing practices.
  9. Exercise discretion, diplomacy, and tact in all interactions.
  10. Knowledge of Electronic Health Record (EHR) and NextGen.
  11. Knowledge of the cultural, social, and economic backgrounds of the Native American Community and other diverse populations.
  12. Proficient in Microsoft Office, including Word, Excel, and PowerPoint.
  13. Ability to function as a member of the integrated health care team of NATIVE HEALTH.
  14. Ability to work with confidential material without betraying the trust of patients, their families, and the agency – HIPAA Compliance.
  15. Ability to assess progress toward program goals and objectives.
  16. Must be dependable, punctual, able to relate well with others, and capable of meeting deadlines.

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. Required: Bachelor’s degree in social work, a related field, or comparable years of experience.
  2. Preferred: Two years of case management or related experience working with Native American and underserved and diverse populations is required.
  3. Experience in domestic violence and sexual assault programs is preferred.
  4. Experience working with EHR systems and other relevant software programs for documentation, scheduling and communication.

Standard Employment Requirements

  1. Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
  2. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  3. Must pass substance abuse testing upon employment and submit to random testing during employment.
  4. Must provide an updated immunization (IZ) record at the time of employment.
  5. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  6. Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.
