The Behavioral Case Manager is responsible for overseeing the coordination of care for individuals with behavioral health conditions, working with a multidisciplinary team to review referrals, coordinate appointments, and support patient practice management. They provide and oversee access to care including appointment communication, appointment adjustments, reminders, confirmation calls, personalized case management, monitor patient progress, ensure adherence to treatment plans, and help patients access necessary resources and services. This role involves collaborating with healthcare providers, family members, and community resources to enhance the overall well-being of the patients they serve.


Responsibilities:

  • Coordinate Care: Serve as the primary point of contact for patients, coordinating care across multiple providers, including therapists, physicians, and community services.
  • Advocate for Patients: Advocate on behalf of patients to ensure they receive timely, appropriate care, and represent their needs and concerns with healthcare providers, insurers, and other relevant parties.
  • Provide Resources: Connect patients to necessary resources, such as therapy, support groups, financial assistance, and housing, ensuring they have the tools needed for their recovery.
  • Offer Support and Education: Provide emotional support, education, and guidance to patients and their families, helping them understand behavioral health conditions, treatment options, and coping strategies.
  • Document and Report: Maintain accurate, detailed, and up-to-date records on patients' cases, including appointment conditions, demographic updates, communication with providers, and any changes in status.
  • Crisis Intervention: Provide connection with providers to offer crisis intervention and stabilization support when needed, ensuring the safety and well-being of the patient during emergency situations.
  • Work with Community Resources: Collaborate with community-based organizations and resources, ensuring that patients are connected to services that complement their treatment plans.

Required Skills/Abilities:

  • Clinical Knowledge: Understanding of behavioral health conditions and treatment methods.
  • Case Management: Ability to coordinate services, and monitor patient progress.
  • Communication: Strong verbal and written skills for interacting with patients, families, and healthcare teams.
  • Empathy: Compassionate, non-judgmental approach when working with patients.
  • Problem-Solving: Ability to address care barriers and respond effectively to crises.
  • Organizational Skills: Ability to manage multiple patient inventories and schedules.
  • Collaboration: Ability to work effectively in a team, consulting with other professionals as needed.
  • Confidentiality: Commitment to patient privacy and ethical practice.
  • Technology: Familiarity with case management software and electronic health records.

Education/Experience

  • Education: High School diploma. 
  • Experience: 1-3 years in practice support in behavioral health settings, with experience in mental health or substance abuse being highly valued.
  • Certifications: Basic Life Support may be required; Crisis Intervention training is a plus.

Physical Demands: 

  • Sitting/Standing: Primarily office-based, but may require periods of standing, walking, or moving between offices or locations.
  • Travel: Some positions may require local travel to meet patients or collaborate with community resources.
  • Lifting: Minimal to moderate lifting, usually under 10-15 pounds (e.g., paperwork or files).
  • Dexterity: Use of computers and office equipment for documentation and communication.
  • Emotional Resilience: Handling emotionally intense situations may require mental and emotional stamina.
