General Description
The Patient Navigator is the first point of contact for clients accessing medical services at Aakom-Kiyii Health Services (AKHS). This role provides front-line administrative support, manages client intake and discharge, and facilitates communication between clients and healthcare teams. The Patient Navigator ensures a respectful, safe, and culturally supportive experience for all participants.
Role and Responsibilities Duties
- Greet clients and guide them through the intake process
- Complete intake/discharge forms and update client records
- Support transportation coordination related to care access
- Maintain accurate, confidential documentation
- Communicate with healthcare staff and external providers
- Respond to client and family inquiries with professionalism
- Track bed availability and client flow for reporting
- Assist with data entry, filing, and supply tracking
- Maintain a welcoming and organized administrative space
- Participate in training and team planning sessions
Knowledge, Skills and Abilities
- Respectful, calm, and culturally aware in client interactions
- Excellent verbal and written communication skills
- Detail-oriented with strong organizational habits
- Ability to maintain confidentiality and manage sensitive information
- Works well independently and as part of a team
- Familiarity with Piikani Nation governance, culture, and language is an asset.
- Emotionally intelligent and compassionate, especially in crisis situations
Requirements
Qualification
- High school diploma or equivalent required
- Post-secondary education in administration or health services an asset
- Experience with client intake and administrative support
- Proficient with Microsoft 365 and data management systems
- Knowledge of health service environments and confidentiality practices
- Experience working in Indigenous or culturally diverse settings is an asset
