The role you’ll contribute:
Serves as a patient navigator throughout the continuum of care.
The value you’ll bring to the team:
- Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
- Coordinates efforts for treatment and promotes quality improvement and educational efforts.
- Compiles data, tracks outcomes, and makes recommendations for process improvement.
- Provides referrals and supports for follow-up care for patients who are uninsured or underinsured.
- Develops and maintains relationships with physicians, post-acute providers, therapists, patients, and families.
- Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services.
- Ensures seamless transitions across the continuum of care.
- Educates patients and families about post-acute care services and expectations.
- Assists in assessing patients for post-acute care and coordinates the clinical transition to home health, hospice, or other appropriate settings.
- Maintains comprehensive knowledge of managed care and community resources.
- Completes and submits all documentation in a timely manner according to department policy.
- Participates in multidisciplinary rounds, care conferences, and coordination with case management.
- Other duties as assigned.
The expertise and experience you’ll need to succeed:
- Associate's of Nursing Required
- Bachelor's of Nursing Preferred
- One or more years of Work Experience Required
- Registered Nurse (RN) Required
- Basic Life Support - CPR Cert (BLS) Required
