Department Details

Highly motivated professional and supportive team. This is a challenging, but very rewarding role helping patients living with diabetes to be successful in diabetes self care and management. Will routinely collaborate with patient care team members.

Summary

Educates and collaborates with patients on self-care involved in the management of diabetes. Assesses health status and proactively manages diabetes to improve patient outcomes in various settings, including clinic and hospitals.

Job Description

Provides initial and on-going social, cultural and learning needs, along with education, goal setting and behavioral interventions to promote diabetes self-management skills and patient empowerment, including but not limited to, intensive insulin management and diabetes technology. Develops diabetes self-management and support plans. Educates patients/family on healthy eating, medication management, physical activity, glucose monitoring, and pattern management. Demonstrates knowledge of pathophysiology, social determinants of health, and evidence-based practice of scope. Supports quality improvement and documentation to effectively maintain program recognition for an effective Diabetes Self-Management Education program. Collaborates with a multidisciplinary team, referring provider, and others related to diabetes care and support plan. CDCES will maintain certification on insulin pumps per requirements based upon department needs. CDCES may provide more complex management recommendations in collaboration with patient and provider according to approved protocols and level of scope. May provide education or support to community members related to diabetes management or prevention.

Qualifications

Must have a bachelor’s degree in nursing or willing to obtain within three years. Graduate from an accredited nursing program preferred, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) preferred.

Two years of experience as a clinical nurse is required with demonstrated leadership skills. Previous experience and knowledge of Diabetes Management preferred.

Requires current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Must hold and maintain certification as a Certified Diabetes Care and Education Specialist from the National Certification Board of Diabetes Care and Education Specialists or obtain that certification within three years when eligible.
