Position Summary 

Tapestry Hospice is seeking an experienced, compassionate, and highly motivated RN Clinical Liaison to serve as a key hospital-based resource. 

This role is responsible for managing referral and intake processes, assessing patient eligibility, and facilitating timely, compliant admissions to hospice services. The Clinical Liaison acts as both a clinical expert and relationship manager, working closely with hospital teams, physicians, case managers, patients, and families to ensure a seamless transition to hospice care. 

 

Essential Responsibilities 

Clinical Evaluation & Admissions 

  • Receive and respond promptly to referrals from hospitals, physicians, case managers, skilled nursing facilities, and other healthcare providers 
  • Assess patients for hospice eligibility through medical record review, clinical evaluation, and collaboration with physicians 
  • Determine appropriateness for General Inpatient (GIP) level of care, focusing on acute symptom management needs 
  • Facilitate the complete hospice admission process, including obtaining consents, physician orders, and required documentation 
  • Ensure all admissions meet Medicare, CMS, and organizational compliance standards 

GIP Level of Care Coordination 

  • Coordinate and admit patients to GIP level of care within hospital settings 
  • Communicate clinical findings and recommendations with physicians and interdisciplinary teams 
  • Collaborate with care teams to ensure appropriate level-of-care utilization and transitions when needed 
  • Support continuity of care throughout the patient’s GIP stay 

Patient & Family Education 

  • Educate patients and families on hospice philosophy, services, and benefits 
  • Provide clear, compassionate guidance on what to expect from hospice care, including GIP services 
  • Support informed decision-making with empathy, professionalism, and cultural sensitivity 

Relationship Management & Outreach 

  • Serve as the primary liaison between Tapestry Hospice and assigned hospitals/facilities 
  • Make routine on-site visits to hospitals to maintain visibility and accessibility 
  • Build and maintain strong professional relationships with referral partners 
  • Provide education to healthcare teams on hospice criteria, services, and referral processes 
  • Identify opportunities to expand hospice awareness and referral networks 

Coordination & Communication 

  • Act as a central point of contact to ensure seamless coordination between referral sources and hospice teams 
  • Facilitate clear, timely communication across interdisciplinary teams 
  • Ensure smooth handoffs from hospital to hospice care 

Documentation & Compliance 

  • Complete all intake and admission documentation accurately and timely 
  • Ensure compliance with HIPAA, Medicare Conditions of Participation, and hospice regulations 
  • Participate in quality improvement initiatives and adhere to organizational standards 

 

Qualifications 

Education & Licensure 

  • Graduate of an accredited school of nursing  
  • Active, unrestricted Registered Nurse (RN) license in Georgia 
  • CPR certification 
  • Valid driver’s license and current automobile insurance 

 

Experience 

  • 3 years of hospice experience preferred 
  • Prior experience in hospital-based roles, case management, or discharge planning strongly preferred 
  • Experience with hospice admissions and GIP level of care highly preferred 

 

Knowledge, Skills & Abilities 

  • Strong understanding of hospice philosophy, patient eligibility, and GIP criteria 
  • Excellent clinical assessment and critical thinking skills 
  • Proven ability to manage admissions, documentation, and coordination processes 
  • Strong communication and interpersonal skills with the ability to engage patients, families, and healthcare professionals 
  • Familiarity with community resources and healthcare referral networks 
  • Ability to work independently and manage multiple priorities effectively
