Technical Experience: 2 years in clinical healthcare environment.

Physical Activities

• Intermittent hand manipulation required

Intermittent lifting and carrying of 20 lbs.

Role Specific Responsibilities

  • Coordinates multidisciplinary case review immediately following positive findings.

  • Provides support and initiate communication with patients upon cancer diagnosis.

  • Assess patient needs upon initial encounter and periodically throughout navigation, matching unmet needs with appropriate services including registration, physician offices, billing, financial assistance, nutrition, pastoral care, social work, homecare, hospice, transportation, etc.

  • Refers patients to financial counseling service to provide information about health coverage, including private insurance, health care savings accounts, and other publicly funded programs.

  • Serves as liaison between modalities, departments and providers as vehicle of communication.

  • Documents in appropriate EHR so that communication is clear.

  • Facilitates interaction and communication with health care staff and providers.

  • Aligns with physicians to enhance relationship between patients and their physicians by providing pertinent education and support.

  • Assess educational needs of patients and families, and provides information or refers to various health care professionals with permission of treating physician.

  • Provides each patient with cancer (disease and site specific) education information.

  • Identifies personnel in departments involved in care of cancer patients and develop relationships with them; offer educational sessions to inform practitioners of navigator’s role, services provided, and encourage referrals.

  • Participates in quality improvement initiatives, track interventions and outcomes, and reports these timely to Nurse Manager.

  • Monitors the patient’s response to care/services provided and communicate concerns with health care providers. All calls are documented in the appropriate EHR within 24 hours and communicated as appropriate to the treating physician.

  • Assists with identification of cancer patients to be potentially enrolled in clinical trials as required. Serves as a clinical coordinator in all aspects of lung cancer screening program, assisting patients and families in navigating the health care system.

  • Assists physicians in coordination of care of lung screening and nodule clinic patients with SIH Imaging Services.

  • Provides patients with smoking cessation education and resources/referrals as appropriate, coordinates with community benefits – smoking cessation programs & resources.

  • Refers patients to other community resources, as necessary.

  • Provides ongoing navigation for patients receiving low dose CT screening and any follow up or additional testing needed within the recommended time.

  • Reviews and reports all results with ordering physicians.

  • Documents, tracks, and reports accurately all data required internally and externally (Lung Cancer Screening Registry) timely.

  • Communicates written treatment completion information to the Primary Care and/or referring physician of screening results and follow up plan as indicated by physician.

  • Provides ongoing documentation of nurse navigation activities on each patient, including the time involved in these activities.

  • Assists in the development, documentation, and analysis of key performance indicators for program performance to key stakeholders.

  • Collaborates with other department members to facilitate effective scheduling of imaging and interventional procedures as needed.

  • Make financial counseling referrals as needed.

  • Coordinate with Lung Cancer Nurse Navigators upon positive pathology.

  • Manage lung screening dashboards & communicate with ordering providers about outstanding orders or missed patient follow ups.

  • Coordinates, facilitates, & participates in health fairs and health/wellness screening programs for cancer awareness for at-risk populations and general population as needed.

  • Develop knowledge and professional skills through cross-training, literature, and complete required training. Documents clinical competencies on an annual basis.

  • Participates in staff meetings, continuing education programs, in-services, and other clinic staff activities.

  • Complies with policies and procedures protecting patient rights and the handling of confidential information as defined by the Cancer Center’s mission and applicable laws and regulations.

  • Ensures work areas are organized and present a safe, accessible, effective, and efficient environment for employees, patients, and families.

Compensation (Commensurate with experience):

$66,144.00 - $102,523.20
