Job Description

The ideal candidate will be a compassionate and dedicated Registered Nurse (RN) joining our team at Eisenhower Center, a Specialized Residential setting focused on the rehabilitation and habilitation of individuals with Traumatic Brain Injury (TBI), Intellectual and Developmental Disabilities (I/DD), Mental Illness (MI), and Autism Spectrum Disorder (ASD). The RN/Health Educator will provide high-quality care, support, and medical oversight to our residents, ensuring their health and well-being.

Role and Responsibilities

·         Provide comprehensive nursing care to residents with TBI, I/DD, MI, and ASD.

·         Administer medication and treatments as prescribed by healthcare providers.

·         Monitor and assess residents' health status, identifying any changes or concerns.

·         Develop and implement individualized care plans in collaboration with the facility treatment team.

·         Educate residents and facility direct care staff about health conditions, treatments, and self-care strategies.

·         Maintain accurate and up-to-date medical records and documentation.

·         Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure holistic care.

·         Participate in care team meetings and contribute to the development of care strategies.

·         Ensure compliance with all relevant regulations, policies, and procedures.

·         Provide emotional support and advocacy for residents.

·         Assist with daily living activities and promote a safe and supportive environment.

  • Traveling between facilities in Manchester, Ann Arbor and Ypsilanti

 

Qualifications:

·         Required: Current and valid RN license in Michigan.

·         Required: Minimum of 3+ years of nursing experience, preferably in a similar setting.

·         Required: Previous experience with TBI/CMH residential care.

·         Advanced proficiency in documentation requirements.

·         Experience working with individuals with TBI, I/DD, MI, and ASD.

·         Strong clinical skills and knowledge of best practices in nursing care.

·         Excellent communication and interpersonal skills.

·         Ability to work collaboratively as part of a multidisciplinary team.

·         Compassionate, patient, and empathetic approach to care.

·         Strong organizational and time management skills.

·         Proficient in computer skills and electronic medical records (EMR) systems.

·         Valid Driver’s License
