Shift: Days with with evening rotational hours to support education class.
General Summary
Provides group and individual Diabetes Education. Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant.
ResponsibilitiesDuties and Responsibilities
- Provides group education and individual education in Diabetes Education.
- Provides nutritional counseling to clients referred by a provider.
- Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
- Manages blood glucose for patients.
- Manages insulin pumps and continuous glucose monitors for patients.
- Collects program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner.
- Collaboratively develops an individualized follow-up plan with each program participant.
- Conducts a follow-up assessment upon completion of program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
- Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.
- Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
- Participates in peer review process to evaluate performance of other professional staff.

QualificationsQualifications
Minimum Education:
- Associates Degree Registered Nurse Required
- Bachelors Degree Preferred
Work Experience:
- 2 years Relevant experience. Required
Licenses:
- Licensed Registered Nurse Upon Hire Required or
- Registered Nurse Multi State License Upon Hire Required or
- Licensed Dietitian-Nutritionist Upon Hire Required and
- Heartsaver CPR/AED Upon Hire Required
- Certified Diabetes Care and Education Specialist within 3 years Preferred
