Position Summary

The Quality Improvement Program (QIP) Coordinator supports the clinic’s Quality Improvement initiatives by tracking performance measures, conducting patient and staff outreach, and ensuring providers and staff remain up to date on all QIP requirements. This role works collaboratively with the QIP team, Compliance Coordinators, and department managers to ensure clinic-wide success in meeting and maintaining QIP measures.

This position has minimal compliance responsibilities and is primarily focused on quality improvement execution, tracking, communication, and follow-through.



Key Responsibilities

Quality Improvement & Performance Tracking

  • Track, monitor, and report progress on all QIP measures and performance metrics.

  • Maintain accurate QIP dashboards, spreadsheets, and tracking tools.

  • Identify gaps in performance and proactively communicate findings to leadership and departments.

  • Assist in preparing QIP reports for internal review and external requirements as needed.

Outreach & Follow-Up

  • Conduct patient outreach related to QIP initiatives (e.g., preventive care gaps, follow-up needs).

  • Coordinate staff and provider outreach to ensure completion of QIP-related tasks.

  • Follow up with departments, providers, and care teams on outstanding QIP items.

  • Serve as a point of contact for QIP-related questions and updates.
  • Scheduling appointments, tracking records and results

Collaboration & Communication

  • Work hand-in-hand with QIP and Compliance Coordinators to align quality goals and reporting.

  • Collaborate with department managers to ensure staff workflows support QIP objectives.

  • Communicate QIP measure updates, deadlines, and expectations to staff and providers.

  • Support QIP meetings by assisting with data preparation and action item tracking.

Process Improvement Support

  • Assist in developing and refining workflows to improve performance on QIP measures.

  • Help ensure staff and providers are educated and up to date on current QIP requirements.

  • Support clinic-wide quality improvement initiatives as assigned.

  • Strong organizational and tracking skills with high attention to detail

  • Excellent communication and follow-up skills

  • Ability to manage multiple priorities and deadlines

  • Comfortable working with data, reports, and spreadsheets

  • Collaborative team player with the ability to work across departments

  • Problem-solving mindset focused on improvement and outcomes



Education and Experience

  • High school diploma or equivalent required

  • Prior healthcare or medical clinic experience preferred

  • Experience with quality improvement, care coordination, outreach, or performance tracking preferred

  • Familiarity with QIP measures, RHC/FQHC environments, or clinical quality metrics is a plus
