Job Description
Provide as-needed backup coverage services for the Behavioral Health (BH), Nursing Facility Monitoring (NFM), and OBRA programs including mental health clinical assessments, diagnostic services, treatment planning, linking, monitoring, advocating, consultation, and intervention services. This position does require you to live in the Kalamazoo area and to work in person with clients.
Per Integrated Services of Kalamazoo (ISK) guidelines, provides case management services for the BH and NFM programs to mentally ill individuals living in the community (BH - age 55 or over) or in a nursing facility (NFM - all ages) who are at risk of psychiatric hospitalization without on-going support and intervention.
Per OBRA guidelines, completes comprehensive clinical mental health assessments for individuals diagnosed as having a serious mental illness and/or intellectual/developmental disability who are seeking admission to or are residing in nursing facilities.
Qualifications
- The position requires a minimum of a Bachelor’s degree in a human service field plus current state certification or licensure in one of the following Michigan Licensures: RN, LLBSW, LBSW, LLMSW, LMSW, LLPC ,LPC, LLP, LP.
- A minimum of a bachelor’s degree in a human services field and one year of experience with people with mental illness.
- Preference will be given to persons with a combination of mental health experience and working with the elderly for at least two years.
