JOB SUMMARY
The Population Health Specialist works to ensure patients are up to date with their diabetic and routine health screenings. This position supports Hartford HealthCare quality and balanced scorecard initiatives by working to ensure patient gap in care completion, creating and maintaining necessary training documents, and training offices on workflows to close gaps in care.  Performs routine audits to identify training and education opportunities for offices where workflows are not being followed.  This is a central position in Hartford, but does require travel to offices across Connecticut to perform trainings.
