The Dayton Children’s population health team identifies and addresses opportunities to improve the healthcare journey for broad populations of children. It is an interdisciplinary team that provides infrastructure, innovation, and an agile approach to improving care for children while prioritizing value over volume. A key component of this approach is our involvement with Partners For Kids (PFK), an accountable care organization responsible for the improvement of health of approximately 120,000 children on Medicaid throughout West Central Ohio.

The Population Health Specialist plays a critical role in advancing and improving health outcomes across the continuum of care. This position is responsible for coordinating the implementation of population health strategies and initiatives, with a strong emphasis on data-driven decision making, cross-functional collaboration, and alignment with clinical and operational priorities. By identifying opportunities for cross-program collaboration and integration, this individual ensures that initiatives are coordinated, complementary, and aligned.

The Population Health Specialist will work closely with population health service lines, providers, clinical staff, and community partners to ensure population health initiatives are effectively executed, monitored, and continuously improved. The ideal candidate is driven to integrate clinical expertise, quality improvement strategies, implementation science frameworks, data analytics, and the voice of lived experience, to guide teams in developing and executing practical, innovative solutions that lead to scalable, sustainable improvement in patient outcomes. This position reports directly to the Manager of Population Health Integration and provides support across multiple clinical service lines. The population health specialist serves as the primary operational resource for designated population health initiatives. This role is designed as a professional growth opportunity, preparing specialists for advancement into management, quality improvement, or clinical operations leadership roles.

Department Specific Job Details:

Experience
3+ years of experience in healthcare, public health, or population health program coordination.

Skills/Competencies:
Microsoft Office Suiteβ€”Word, Excel, Power Point
Demonstrated experience using data to drive decisions and improve outcomes.
Strong project management and organizational skills.
Excellent interpersonal and communication skills, with the ability to engage diverse stakeholders.
Familiarity with clinical workflows, EHR systems, and healthcare quality metrics is a plus.


Preferred Qualifications

Education:

Bachelor’s degree in nursing preferred

Skills/Competencies:
Proficiency in data visualization tools (e.g., Tableau, Power BI).
Understanding of social and structural factors that impact health outcomes.
Strong understanding of population health best practices.
Experience working in a matrixed environment with clinical and non-clinical teams.
