POSITION SUMMARY
The Population Health Specialist (PHS) is responsible for the day-to-day activities of Little River Medical Center (LRMC) to improve population health outcomes and quality of care. The PHS serves in an expanded role to collaborate with and support primary care physicians and their support staff to identify and close HEDIS care gaps as part of quality incentives with managed care contracts with the goal of optimizing our patients’ health and wellbeing. The PHS will also perform Community Health Worker (CHW) duties as needed through an integrated approach to care management and community outreach.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
  • Perform day-to-day activities of LRMC to improve population health outcomes, quality of care, and care value across all LRMC sites.
  • Participate as a key member of the LRMC QI Team to identify patient care gaps and work with providers and patients to close gaps in care..
  • Develop/acquire knowledge and understanding of HEDIS and UDS measures for improving quality.
  • Participate in “deep dives” to identify patients who are high cost/high utilizers and provide appropriate outreach, education and support to identified patients to help control health care costs.
  • Outreach to patients recently hospitalized and/or treated in the Emergency Room for aftercare and follow-up with LRMC’s PCP. 
  • Educate patients on appropriate ER utilization and how LRMC can meet their medical needs.
  • Participate in quality initiatives, campaigns, or special events as assigned.  Conduct patient outreach including communication to promote preventative health to ensure quality care of our patients.
  • Outreach to individuals assigned to LRMC as their PCP and assist with access to care, services, and treatment including linkage to patient centered medical home at LRMC.
  • Provide clinical support and assistance with Diabetic Eye Exams, Colorectal Cancer Screening FIT tests, Diabetic Monitoring programs and other health and preventative screenings as assigned.
  • Act as a key resource to PCP with patients who have chronic diseases.  Coach patients in effective management of their chronic health conditions, medication adherence and self-care.
  • Perform chart reviews and data mining to obtain test results, lab results, and medical documentation that will close gaps in care. 
  • Facilitate communication and coordinate services between providers.
  • Build and maintain positive working relationships with the patients, providers, nurses, agency representatives, supervisors, and office staff.
  • Continuously expand knowledge and understanding of community resources, services, and programs in the community as well as within LRMC.
QUALIFICATIONS:
  • Applicant must be able to perform all of the requirements of the essential job functions; must have skills required to promote team building with the staff, must be able to comply to all policies and ethical business practices; must be able to perform all required duties described and perform up to the standards of LRCM and observe all federal compliances; must meet physical demands of the job.
  • Must have basic computer skills.
EDUCATION and/or EXPERIENCE:
  • High School Diploma or its equivalent.
  • Experience working in a multicultural setting.
  • Experience working in a medical office or community-based setting for at least 1-2 years preferred.
