PCHH- Behavioral Health Consultant

Pay: Base Salary $95,000

 * Sign-on Incentive Available * 
$5,000 upon hire
$5000 after 6 months
$5000 after 12 months
3-year commitment and repayment to be pro-rated.

Essential Functions:

• Will support the care team in identifying and behaviorally intervening with patients to improve their physical health condition
• Will assist individuals in improving their health status and managing chronic illnesses. 
• will assist with medication adherence, treatment plan adherence, self-management support and goal setting, and facilitate group classes. 
• Interventions for individuals with behavioral health problems.
• Coaching sessions

Skills Abilities:

• Ability to communicate clearly and effectively with co-workers and patients. 
• Ability to complete competency CPR and First Aid
• Ability to prioritize and compete multiple tasks simultaneously.
• Effective oral and written communication skills.

Working Conditions:

• Possible exposure to inside environmental conditions.
• Exposure to distressed patients, families, or visitors.

Physical Demands:

• Requires standing, sitting or walking for up to 8 hours or longer per day.
• The ability to lift up to 50 lbs.

Qualifications:

• LCSW, LMSW, LPC's: LMFT, Clinical Psychologists and/or individuals working towards certification and are currently in clinical supervision.
