Patient Navigator- Part Time, Days, 8 AM - 4 PM, Atlantic Health Morristown Medical Center

1. Responsible for identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health. 
   
   
   Principal Accountabilities:
   
   
   1. Investigates available community resources and provides resources and information to reduce barriers to care and help patients access important services. 
   2. Supports patients and their families through the treatment journey with information and resources. 
   3. In collaboration with the supportive care team, navigates patients through the available internal and external resources, programs and services. 
   4. Educates and supports each patient empowering them to make informed decisions and access services. 
   5. Remains a support system throughout the patient's treatment. 
   6. Performs other related duties as assigned.

Required:
1. High School Diploma or equivalent.
2. Strong computer acumen
3. Experience and knowledge of all aspects of the healthcare system.
