Fauquier Health is proud to offer a wide range of screening and diagnostic imaging services to provide customized care for each individual patient’s needs. As a Patient Navigator, you will work in collaboration with the multidisciplinary care team to ensure effective communication, support quality outcomes, and empower patients to become active participants in their own care. This position will serve as a liaison throughout the facility and within the community offering clinical expertise, education, consultation, and advocacy on behalf of this patient population. Initial efforts will focus on supporting patients with colon, urologic, and breast cancer diagnoses with plans to broaden navigation support as service needs evolve.
