Job Summary:

Assists patients to navigate and guide through the healthcare system to result in timely care.  Works under the direction of the Director of Care Management and in collaboration with multidisciplinary and medical teams to deliver quality patient care services. Helps to coordinate patient care, connect patients with resources, and help patients understand the healthcare system while working collaboratively with multidisciplinary and medical teams to deliver quality patient care services. Monitors patient outcomes and participates in quality improvement activities. Works alongside management staff to implement and develop established or new programs related to department efficiency and cost effective program implementation as well as practice growth.

 

Minimum Job Requirements:

 

Work Experience

Three years of experience as a Registered Nurse required; or six years of professional experience in social work, preferably in a hospital setting.

 

License/Registration/Certification

Social Work License (LMSW) in the State of Texas; or Current Texas Registered Nurse License.

 

Education and Training

Bachelor Degree in Nursing required; or Master’s degree in Social Work required. 

 

Skills

  1. Excellent interpersonal communication skills.
  2. Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.
  3. Strong customer service skills.
  4. Proficient in computer skills and programs such as Excel, Microsoft Word, PowerPoint, etc.
  5. Current working knowledge of hospital services.
  6. Able to work with people of all social, economic, and cultural backgrounds.  Culturally sensitive.
  7. Flexible, open-minded and adaptable to change.
  8. Knowledge and understanding of available community resources.
  9. Strong organizational and time management skills, as evidenced by capacity to prioritize multiple. Ability to exercise sound judgment in interactions with patients/families.
  10. Ability to work independently and exercise sound judgment in interactions with physicians, patients, and families.
  11. Bilingual English/Spanish preferred.
  12. Ability to read and comprehend English at a level necessary to follow written and oral instructions and safety precautions.
