The PACE Social Work (MSW) Supervisor plays a critical and progressive leadership role in building, scaling, and sustaining a high-performing social services program that supports frail older adults to remain safely in the community. This role ensures the delivery of high-quality, participant-centered care across the continuum, while supporting, supervising, and developing social services staff in alignment with PACE regulations, organizational policies, and interdisciplinary care model standards. The Social Work Supervisor (MSW) is accountable for optimizing wrap around services, community supports, and caregiver resources to honor what matters most to PACE participants.Essential Responsibilities:
Scale, grow, and lead a team of social work professionals as the PACE organization progresses
Actively participate and collaborate as a member of the Interdisciplinary Team, including care planning.
Provide leadership to PACE social workers to ensure high-quality care. Serve as a mentor to enhance the knowledge and skills of the social work team and act as a clinical supervisor to MSWs.
Conduct orientation and training for new hires. Provide ongoing supervision, training, coaching, evaluation, and leadership to assigned team members, including annual reviews, promotions, and terminations.
Ensure all services are provided in a manner consistent with State and Federal PACE regulatory requirements and that the team understands these requirements.
Incorporate knowledge of comorbidity and established standards of practice associated with medical and behavioral health diagnoses.
Accurately and completely document in the electronic health record in a timely manner.
Serve as a resource to assist participants and their families in accessing services, facilities, and community resources for their healthcare needs.
Establish and maintain relationships with community services, and internal and external referral sources.
Provide on-call coverage for direct patient care with other staff as needed.
Identify and designate ongoing training needs for social work staff and develop/implement appropriate educational offerings.
Ensure exceptional customer service to participants of the PACE program.
Report on and collaborate to resolve participant grievances.
Work collaboratively with other PACE leaders to maintain and develop departmental communication materials, training/educational materials, and policies and procedures.
Actively participate in various strategic and internal committees to disseminate information within the organization.
Actively participate in efforts to support compliance and regulatory requirements across lines of business.
Coordinate business activities by maintaining collaborative partnerships with key departments.
Monitor, evaluate, and act on team assignments relating to volume, timelines, accuracy, customer service, and other quality and performance measures as appropriate.
Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
Supporting Responsibilities:
Meet department and company performance and attendance expectations.
Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.

SUCCESS PROFILE
Work Experience: A minimum of 5 years of social work experience, including at least 1 year of leadership experience and 1 year of experience working with a frail or elderly population, is required. Experience in PACE or other Long Term Services and Supports settings is preferred.
Education, Certificates, Licenses: A Masterβs degree in Social Work (MSW) is required. A Licensed Clinical Social Worker (LCSW) in the State of Oregon is preferred. Valid CPR certification required. A valid Oregon driverβs license is required. Relevant behavioral health care experience is preferred.
Knowledge: Employee(s) must have knowledge of behavioral health State Medicaid and Medicare regulations. Strong analytical and problem-solving skills are essential. Proficiency with computer information systems and software is required, along with basic statistics and research methods.
Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:
Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
Only act within the scope of his or her authority to practice.
Have 1 year of experience working with a frail or elderly population
Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
Be medically cleared for communicable diseases before engaging in direct participant contact.

Physical Requirements:Β Work is performed in a clinic setting with typical healthcare exposures, including contact with bodily fluids, infectious diseases, and cleaning chemicals; PPE is provided and required. The role involves stooping, bending, sitting, standing, and moving between clinical areas for extended periods. May assist participants with mobility, including guiding, supporting, escorting, and occasional transfer assistance using proper body mechanics. Must be able to lift, push, pull, or carry up to 35 pounds occasionally and perform frequent repetitive motions such as typing and operating medical or office equipment. Some positions require occasional travel to PACE sites, participant homes, or external appointments. Must be able to speak clearly, understand verbal instructions, and interact professionally with participants, caregivers, and staff.
