Position Function Summary
Under the direction of the Medical Records Coordinator, the Medical Records & Health Information Technician maintains the confidential patient medical records for release to LFMC providers and external agencies. Receives correspondence for inclusion into the medical record to insure accurate and timely information is available to all providers and patients.
Essential Responsibilities
General Responsibilities
- Releases medical information upon client’s and clinician’s request according to LFMC’s protocol;
- Responds to requests for patient records, both within the facility and by external sources, retrieving them and transmitting them appropriately;
- Maintains a record of requests for information and transfers;
- Works with Patient Services staff to change and update patient information by submitting computer source documents with new patient information as assigned;
- Responsible for safeguarding patient records and ensuring compliance with HIPAA standards;
- Ensures electronic medical records are assembled in standard order, and are accurate and complete;
- Performs audits of medical records as assigned;
- Updates and corrects patient information that may be scanned or labeled incorrectly in patient documents;
- Reviews documents in patient electronic medical record for completeness and accuracy;
Clerk Responsibilities
- Scans x-ray results, external agency records, and other reports into the patient medical record according to LFMC electronic medical record format and protocol ensuring they are completed in an accurate and timely manner;
- Maintains records in on-site storage, retrieves charts as needed following LFMC’s request protocol;
Preparation Specialist Responsibilities
- Maintains an accurate record of upcoming prepped appointments and promptly preps any new appointments that may be added to providers’ schedules;
- Obtains x-ray results, referrals, external agency records, and other reports for the patient medical record prior to an upcoming patient visit according to LFMC format and protocol;
- Obtains/retrieves records from Christus St. Vincent Regional Medical Center system for hospital, ER, labs and imaging reports;
- Performs PSR responsibilities as needed, such as answering phones, faxing, and patient check in and out;
- Other duties as assigned.
Education, Experience and Skills
- High School graduate or equivalent required.
- Basic knowledge and ability of general office procedures including filing, copying, faxing, use of a computer, to multitask within established deadlines, work under the direction of a supervisor and follow instructions for work completion, and take initiative to resolve patient concerns and problems.
- Ability to read and interpret documents such as medical records, write at a moderate level of competence, and communicate effectively;
