The Hospice Clinical Liaison is responsible for maintaining and promoting relationships within the Eden Hospice service area and evaluating patients for possible admission to hospice care.

Job Summary:
- Responsible for maintaining relationships with referral sources including, but not limited to hospitals, long term care facilities, rehabilitation centers, and physician practices.
- Works closely with agency staff and referral sources to guarantee effective communication and safe transition of referrals.
- Obtains informed consents and provides nursing assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life.
- Makes visits to potential hospice patients (in both facilities and home settings), performs pre admission evaluations for appropriateness for hospice services and initiates the admission process by educating and informing patient/family of hospice and its’ services.
- Explains Eden Health’s program to patients, family members, and other referral sources in an accurate, complete, and thorough manner.
- Demonstrates knowledge of hospice admission criteria, regulatory requirements, reimbursement guidelines and agency policies and procedures.
- Provides accurate and timely documentation of all marketing activities back to the agency as requested.
- Follows-up on referrals for evaluation and/or admission to Eden Health program.
- Attends and participates in in-services, staff meetings, and case conferences.
- Completes all documentation in an accurate, timely, and legible manner.
- Obtains outstanding orders or documentation as requested by agency.
- Ability to communicate effectively with patients, coworkers, and other healthcare professionals.
- Participate in the development and implementation of processes in accordance with agency’s evolving goals.
- Other duties as assigned.
Education/Experience Requirements:
- Graduate of an accredited nursing program.
- Current Registered Nurse licensure in the state of employment.
- Experience as an RN commensurate with one (1) of the following:
- One (1) year acute care experience within the last two (2) years.
- One (1) year home health care experience within the last two (2) years.
- One (1) year community health or public health experience within the last two (2) years. d. One (1) year hospice experience within the last two (2) years.
- Current BCLS certification.
- A valid driver’s license, reliable auto, and current auto insurance.
- Able to communicate effectively in English verbally and in writing
- Working knowledge of computer software programs
- Able to pass criminal background check and national sex offender clearance
- Must be able to physically navigate through community for entire shift. This is not an in-office administrative sales position.
- Self-directed and can work in the field with minimum supervision.
