Patient Navigator / Navigator Liaison (LPN or RN)

At Dignity Care Hospice, part of CommuniCare’s family of companies, we are committed to providing exceptional patient-centered care and support to patients and families during their life journey. We are seeking a compassionate Patient Navigator / Navigator Liaison to serve as a bridge between patients, families, and our internal healthcare system.

The ideal candidate will possess a strong translatable clinical skill set, an ability to have excellent communication, and a dedication to guiding patients and families through care planning, treatment decisions, and available resources externally and internally.

Role Overview

A Navigator Liaison, often referred to as a patient navigator or clinical trials navigator, plays a vital role in supporting patients throughout their healthcare experience. This role ensures patients understand their conditions, treatment options, and available resources, while also facilitating communication between patients and healthcare providers to empower informed decision-making.

Key Responsibilities

  • Conduct comprehensive assessments of patients to evaluate medical, emotional, and social needs.
  • Collaborate with interdisciplinary teams (physicians, social workers, therapists, etc.) to develop individualized care plans.
  • Assist patients and families with advanced care planning, including goals of care, treatment preferences, and end-of-life decisions.
  • Serve as a liaison between patients, families, and providers to facilitate smooth care transitions.
  • Educate and support patients and families on available hospice and healthcare resources.
  • Monitor patient progress and adjust care recommendations as appropriate.
  • Maintain accurate documentation of assessments, care plans, and interactions in compliance with regulations.
  • Participate in quality improvement initiatives and help develop best practices in patient navigation.
Qualifications
  • LPN or RN with a valid  nursing license.
  • Minimum of 3 years of clinical nursing experience (home health, hospice, or skilled nursing preferred).
  • Strong knowledge of advanced care planning and healthcare regulations.
  • Excellent interpersonal and communication skills with the ability to build rapport with patients and families.
  • Ability to work both independently and collaboratively with interdisciplinary teams.
  • Strong organizational and time-management skills.
Preferred Qualifications
  • Experience with electronic health records (EHR) systems.
  • RN licensure (preferred, but not required).
Work Environment
  • This position requires travel to various healthcare facilities and patient homes within the service area.
  • Flexibility in work hours may be required to meet patient needs.
