Full Time | Monday through Friday | Day Schedule

Phoenix Home Care & Hospice is seeking a detail oriented Home Health Clinical Reviewer to join our Independence team. This role is essential in ensuring clinical documentation is accurate, compliant, and supports high quality patient care.

What You Will Do

Review clinical documentation for accuracy, completeness, and compliance
Ensure documentation meets state, federal, and payer requirements
Audit charts and identify areas for improvement or correction
Collaborate with clinicians to resolve documentation issues and provide guidance
Support quality assurance and performance improvement initiatives
Assist with OASIS review and documentation standards as applicable
Maintain organized records and meet deadlines for chart review and submission
Communicate effectively with clinical and administrative teams

What We Are Looking For

Active Missouri RN or LPN license required
Home Health experience required
Experience with clinical documentation review or QA preferred
Strong knowledge of home health regulations and compliance standards
Attention to detail and ability to work independently
Strong communication and problem solving skills
Comfort working in a fast paced, deadline driven environment
Experience with EMR systems and OASIS a plus
