At NYP, Health Priorities Specialists spend their days navigating a multitude of adventures: on the phone, on the floor, dashing across the hospital and diving into the paperwork. They might be the first person a patient sees and last person to smile and wave. But every minute, they're always Putting Patients First.

Health Priorities Specialist - Patient Navigator - Evenings - Spanish Speaking

Serve as a patient ally throughout the continuum of care. Coordinate the full breadth of the patient experience, from E.R. to discharge. Then, go a step beyond by identifying patients with symptoms across recurring episodes and direct them to the specialist or service they need. Tap into community resources, like neighborhood groups, and up the ante on patient education for health issues seen throughout New York. 

The schedule for this position will be Tuesday - Saturday from 3:00 pm - 11:30 pm. 

Preferred Criteria

• Clinical background

• Diverse and culturally sensitive customer service skills 

Required Criteria

• Bachelor's degree 

• Fluent in Spanish

• 2-3 years experience in primary or community health

• Knowledge of overall hospital and community resources
