Job Description
This position will support the TTUHSC SON Combest Wellness Center in Lubbock, Texas as a Care Manager.  In this position employee will serve as a member of the interdisciplinary care team and is responsible for providing care management for clinic patients. Will work with a diverse patient population. Duties will include, but are not limited to, classifying and implementing health education programs designed to increase awareness and knowledge for the improvement of health in a single, group, or community setting. Keeps up-to-date on changes in health care technology to keep health education program material current. Employees in this classification receive limited supervision and work is evaluated based on progress toward a set of goals by immediate supervisor. May supervise student assistants and other clerical personnel. Work is performed under usual clinic and office conditions and requires a great deal of patient contact and contact with community resource people. Some work is performed after usual office hours.

Major/Essential Functions
This position will support the TTUHSC SON Combest Wellness Center in Lubbock, Texas as a Care Manager.  In this position employee will serve as a member of the interdisciplinary care team and is responsible for providing care management for clinic patients. Will work with a diverse patient population. Primary responsibilities in this role include:
  • Assist patients through the health care system while supporting self-management of disease.
  • Provide patient education in self-management.
  • Assist patient in understanding their plan of care and anticipated outcomes.
  • Coordinate with community partners and other health care entities for continuity of care.
  • Participate in community outreach and sponsored community events.
  • Participate in data collection to help monitor health outcomes.
  • May supervise student volunteer or student clinical experience.
  • Attend required meetings.
  • Assist referral specialist with referral management as needed.
  • Assist with tracking and follow-up on cancer screenings and diagnostic testing.


  • Community Health Worker Certified -- required.
  • Experience working with electronic medical records (EMR).
  • At least 2 years’ experience working with patients in a medical or ambulatory health care setting.
  • BLS certified.
  • Bilingual (English/Spanish).


Pay Statement
Compensation is commensurate upon the qualifications of the individual selected and budgetary guidelines of the hiring department, as well as the institutional pay plan. For additional information, please reference the institutional pay plan website at https://app4.ttuhsc.edu/payplan.

  • In this role, the employee may be asked to participate in health fairs, community events, and civic engagement. 
  • May travel to attend conference(s).

Required Qualifications
Bachelor's degree from an approved accredited university in a field relevant to the work being conducted; one year of work experience demonstrating health education practices, techniques, and principles.
