Qualifications:
Graduate of an accredited school of nursing.                                                                                                                                  
Current license to practice as a Licensed Practical Nurse in the State of Ohio.                                                             
One to three years full-time experience in nursing practice is preferred. 
 
Skills/Abilities:  
Ability to work as a team member.
Computer experience desirable.
Ability to work with a culturally diverse group of people
Must be willing to work flexible hours as required by position.
Must be willing to travel to other HPWO locations.
 
Essential Functions and Basic Duties:
  • Performs other duties as assigned.
  • Attends staff meetings.
  • Assists with conducting assessments and surveys to determine health education needs of staff.
  • Assists with assessing the health needs of health center patients.
  • Assists with the development of materials and trainings to teach staff health related topics, such as managing chronic conditions, medication adherence, and the importance of keeping follow-up visits.
  • Collaborates with CNO, DON and ADON’s to prioritize staff development needs.
  • Monitors staff education and recommends direction with attention to health literacy.
  • Evaluates the effectiveness of educational trainings with support staff.
  • Participate in patient and family communication committee meetings.
 
Work Environment:
Prolonged sitting or standing may be required. The physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with duties in a clinic office setting will be performed on a repetitive basis. This also includes bending, twisting, reaching, lifting, pulling, pushing, and walking. The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals, fax machines, and copiers is required. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
