Your Role
The Federal Employee Program (FEP) team performs integrated case management (CM) and disease management (DM) activities in collaboration with members and those involved with members’ care including registered nurse case managers, social workers, dietitians, health coaches, pharmacists, and physicians. The FEP Health Coach will report to the FEP Department Manager/Supervisor. In this role you will be responsible for developing a wellness coaching relationship and track improvements based on health outcomes. You will collaborate with the CM team to help members achieve optimal health and wellness goals with emphasis on HEDIS measures.
Your Work
In this role, you will:
Support operational aspects of the FEP team to meet the organization’s quality of care metrics such as HEDIS measures for areas like: follow-up post hospitalization, routine colorectal / breast cancer screenings and well-child visits / immunizations
Encourage adoption of habits that are conducive to a higher quality of life
Outreach eligible members for enrollment into the program and provide necessary wellness coaching to reduce or eliminate high-risk behaviors
Provide approved health education materials to program participants via online resources or by email
Your Knowledge and Experience
Requires a bachelor’s degree.
Requires at least 5 years of prior relevant experience.
Experience in health promotion, health education, athletic training, nutrition, other health related field or counseling and two years of experience as a wellness and/or health coach. Bachelor’s degree or higher preferred.
Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, prenatal/postnatal care, stress reduction, and chronic conditions such as diabetes, CAD, asthma, COPD, CHF, and cancer.
Certification from an accredited professional health or wellness coach training program preferred.
Must be able to sit for extended periods of time to provide telephonic outreach and coaching.
Must be proficient in operating PC-based software programs and demonstrate competency and knowledge of Windows based programs and EMR documentation systems.
