Caseload
The Health Coach will be responsible for their own caseloads, while working collaboratively with the wider team.
Mental health and addiction
- When working with whānau, drawing upon mental health training, lived experience, ngā piripoho o Te Arawa Whānau Ora and other health models of practice.
Best Practice - Best practice is delivered drawing upon ngā tūāpapa o Te Arawa Whānau Ora and other health models of practice.
- Support whānau resilience and self-determination.
- Seek, find, and address opportunities to improve quality of service for Māori.
- Fortnightly caseload reviews are attended by all ensuring growth, education and advancement for all whānau is at the forefront.
Record Keeping - Timely and accurate notes and data are recorded in the patient management system.
Workshop Deliver at least one workshop monthly focused on prevention activities, being well and staying well including (but not limited to):
- Lifestyle changes including addictions support
- Healthy eating, kai sovereignty and tupuna kai
- Physical activity
- Whakapapa
- Taiao
Events Assist with the delivery of events including (but not limited to)
- Whānau and Community events
- Health promotion activities
- Annual symposium
- Pop-up screening clinics
Whānau plans - All mahi is whānau led and documented through a whānau plan and goals are in place for all.
- All whānau have the opportunity to have an aspirational plan.
Internal collaboration - Collectively deliver an integrated approach to provide wrap around support and opportunities for whānau.
External collaboration - Maintain strong relationships across all general practices and local service providers.
- Contribute and participate in networking and promotion of Matariki services and Te Arawa Whānau Ora.
Reporting - Document success stories and opportunities for improvements, to contribute towards internal and external reporting.
- Provide reports as requested.
