The right support at the right time can change someone's health journey.
As a Health Coach, you'll work alongside people and whānau to help them take practical steps towards better health and wellbeing. You'll listen, encourage, build confidence and help people turn their goals into meaningful, achievable change.
At Tāmaki Health, you'll be part of our Wellness Support Team - working closely with GPs, nurses, Health Improvement Practitioners and other health and wellbeing professionals to provide connected, person-centred care.
We're looking for an experienced and qualified Health Coach to join our team.
About the role
This is a permanent, full-time position working 40 hours per week.
You'll provide coverage across our Mt Roskill and Dawson Road clinics, with future onsite coverage at our Sandringham clinic.
The role involves variable shifts, including weekend work, so you'll need to be flexible and comfortable working across different clinic environments and schedules.
As a Health Coach, you'll work as part of the everyday primary care team, supporting patients and whānau with their health, wellbeing and long-term goals.
You will:
- Provide brief, targeted Health Coaching to individuals and whānau.
- Support people to manage long-term conditions and improve their health and wellbeing.
- Help patients identify meaningful goals and practical steps towards achieving them.
- Work closely with GPs, nurses, Health Improvement Practitioners and the wider multidisciplinary team.
- Build strong relationships with patients, whānau and clinic teams.
- Support wellbeing as part of routine primary care.
- Work across different clinics as service needs require.
- Contribute to group programmes and wellbeing initiatives where required.
- Work independently while remaining connected to your wider multidisciplinary team.
- Take an active approach to ongoing learning and professional development.
What you'll bring
To be considered for this role, you will need:
- A relevant Bachelor's degree in a health, wellbeing or related field.
- Completed Health Coach courses, training or recognised Health Coach certification.
- Previous work experience in a relevant health, wellbeing or related field.
You'll also bring:
- Strong interpersonal and communication skills.
- The ability to build trust and develop meaningful relationships with people from diverse backgrounds.
- A collaborative approach and the ability to contribute positively to different clinic teams.
- The ability to work independently, manage your time and adapt to changing priorities and environments.
- An agile and flexible approach to work.
- A genuine commitment to self-directed learning and ongoing professional development.
- Confidence using computer systems and a willingness to learn new technology and ways of working.
- Cultural awareness and the ability to work effectively with Auckland's diverse communities.
- A genuine passion for supporting people to improve their health and wellbeing.
We're looking for someone who is comfortable with variety, enjoys being part of a team and can also take ownership of their own work and development.
About the Wellness Support Team
Tāmaki Health's Wellness Support Team brings together a multidisciplinary group of professionals providing accessible, integrated wellbeing support within primary care.
Our team works alongside patients and whānau across mental, physical and social wellbeing. Health Coaches play an important role in supporting people to make meaningful and sustainable changes to their health.
You'll work alongside and be supported by a wider network of Health Coaches, Health Improvement Practitioners, psychologists, therapists and other wellbeing professionals, while building strong relationships with the clinical teams within our practices.
The Wellness Support Team was established in 2012 and has been recognised for its innovative approach to primary healthcare, including winning Best Mental Health Programme at the New Zealand Primary Healthcare Awards.
