Full-time, regular position in the Diabetes Wellness Center in Talihina | Schedule: Mon-Fri 8am-4:30pm | Pay: Hourly, dependent upon experience 

Weekly earned wage access is an option for this position 

Job Purpose or Objective(s):  The Diabetes Educator provides nursing assessment, care, and education to diabetic patients and their families. You will report to the Manager or Director of your department.

 

Primary Tasks:

  1. Provide diabetes self-management education including healthy coping, healthy eating, problem solving, being active, taking medication, monitoring, and reducing risks.

  2. Complete comprehensive assessments for each patient, interpret personal health data, and develop an individualized plan of care based on needs and goals.

  3. Educate patients on the use of CGMs and insulin devices; assist outlying providers with CGM orders and device downloading.

  4. Assess patients at various stages of diabetes, with focus on prevention of complications and management of stable diabetes. 

  5. Provide on-going assessment and recommendation to the treatment team, patient, and family. 

  6. Help patients in monitoring blood glucose and adjusting medication.

  7. Ensure patients are scheduled for annual appointments; assist with walk ins and other diabetes related appointments.

  8. Document all assessments and interventions, including recommendations and goals in EHR.

  9. Perform other responsibilities as may be assigned.

 

Required Education and Experience: 

  • Associate degree
  • Current RN licensure with the Oklahoma State Board of Nursing or Multi-State RN License
  • Current BLS certification
