About the Role
The Diabetes CNC is a senior nursing leader and independent practitioner who drives excellence in diabetes care across the Royal Victorian Eye and Ear Hospital. This role blends advanced clinical expertise with education, innovation and service development to strengthen the safety, quality and experience of care for patients with diabetes.
Working closely with medical, nursing and allied health teams, the CNC delivers expert, patient‑centred diabetes management and empowers patients, families and carers with the skills and confidence to self‑manage their condition. The role also provides high‑impact diabetes education to staff across all disciplines, building capability and consistency in best‑practice care.
The CNC leads the design and evaluation of diabetes education programs, clinical initiatives and new models of care aligned with best‑practice guidelines and the Eye and Ear’s strategic priorities. The role ensures timely, coordinated referrals within the hospital and to external providers, supporting a seamless, integrated care experience. Through advanced clinical judgement and evidence‑based practice, the CNC contributes to improved outcomes and ongoing enhancement of diabetes services across the organisation.
