We are seeking an experienced Program Manager to oversee and manage One Community HealthοΏ½s health-related social needs (HRSN) case management, our Connected Care program providing in-home support to patients who are 55 and older, and our Peer Support Specialists. The Program Manager will ensure that Community Health Workers (CHWs) provide comprehensive support to patients and their families, connect patients to available resources and community services, and collaborate closely with primary care providers to ensure continuity of care. The role also includes overseeing documentation practices and managing team performance to meet each programοΏ½s goals.

Qualifications:

  • Associates degree in Nursing, Social Work, Community Health, Public Health, or a related field required. bachelors or higher degree preferred.

  • Minimum of 5 years of experience in healthcare, case management, or social work

  • At least 2 years of leadership experience managing teams

  • Experienced in patient care coordination and resource navigation

  • Experience with community health programs, in-home care services, community mental health and/or substance use treatment settings is preferred

Skills:

  • Excellent leadership, team management, and interpersonal skills.

  • Strong communication and organizational skills, with the ability to manage multiple priorities simultaneously.

  • Understanding of recovery principles, trauma-informed care, and person-centered practices.

  • Proficient in electronic health record systems and healthcare documentation.

  • Ability to work collaboratively across multidisciplinary teams and foster a supportive and collaborative team environment.

Compensation:

  • Exempt: starting at $70,000 annually, depending on experience.
  • Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
  • Milage Reimbursement: In accordance with OCH policy.
