GENERAL STATEMENT OF DUTIES

Performs managerial, coordinative, and administrative work in planning and implementing a comprehensive agency health education program.

DISTINGUISHING FEATURES OF THE CLASS

Employees in this class provide leadership and direction to other community health educators in developing, organizing, and implementing a health education program to help meet the particular needs of the community as defined by the mission and overall organizational objectives and programs.  Work includes providing overall direction and input into the agency strategic plan, projecting and providing resources for program development, monitoring and evaluating program accomplishments and assuring that community needs are being met, and providing input into program and policy directions.  Work requires an expansive knowledge of the health education fields, comprehensive knowledge of community resources, and leadership and supervisory skills.  Work is performed under the general direction of the agency director and is reviewed through conferences, reports, observation of program activities completed, and feedback from groups receiving the services.

DUTIES AND RESPONSIBILITIES
Essential Duties and Tasks

The purpose of the Community Health Educator III (Social Determinants of Health) is to provide oversight and leadership for the Minority Diabetes Prevention Program and related initiatives that address health equity and social determinants of health (SDOH), including the Community Health Assessment (CHA). This role contributes to reducing health disparities and improving outcomes for historically marginalized and underserved populations in Guilford County via the management of data collection and analysis and coordination of collaboration with local leaders, coalitions, and organizations to improve health outcomes for high-risk populations.

The Community Health Educator III manages the planning, execution, and assessment of the Social Determinants of Health (SDOH) and Violence Prevention projects aimed at improving health outcomes for underserved and marginalized populations within the communities of Guilford County. Provides resources, health education, and referrals to communities for various priority health and social needs. Supports the coordination of the Minority Diabetes Prevention Program (MDPP) and health communities. Provides statistical, evidence-based, and technical resources to support the completion of activities related to the Community Health Assessment (CHA), Community Health Improvement Plans (CHIP), and other related activities.

Collaborates with community-based organizations, health care networks, social services agencies, and other key stakeholders to identify and address social, economic, and environmental factors impacting health outcomes in Guilford County communities. Conducts meetings with community partners; writes and/or reviews monthly and/or quarterly reports; serves as liaison between the Health Department and state and regional departments such as the NC Office of Violence Prevention, NCDOJ, NCDHHS, and the CDC (Minority Diabetes Prevention Program).

Performs site visits with subcontractors or CBOs such as One Step Further (Gate City Coalition); plans, attends, and participates in community-facing activities, events, and meetings to gather insights into the community’s perspective and to address local SDOH needs.

Utilizes local and state data, along with evidence-based practices, to create and implement strategies that effectively address social determinants of health and their impact on populations within Guilford County communities. Participates in data briefs and meetings conducted by collaborating LEAs, CBOs, and other departments. Supports program implementation and evaluation with appropriate data analysis and reporting.

Supervises, coordinates, and facilitates annual training for staff on social determinants of health, health disparities, and health equity to enhance organizational knowledge and capacity.

Collaborates with key health department staff, including the Epidemiologist, Community Health Educators, and community partners, to complete activities of the Community Health Assessment (CHA), Community Health Improvement Plans (CHIP), and action planning activities aligned with the health department’s accreditation standards.

Coordinates and facilitates activities related to the coalition, Guilford County Violence Prevention Initiative, that addresses systemic causes of violence, including social determinants of health such as lack of access to health care and economic instability. Participates in Health Department administrative activities as assigned by the Health Director and Program Manager, including those related to measles and other public health emergencies.

• Considerable knowledge of modern educational theory and techniques in the field of public health education.
• Considerable knowledge and skills in the approaches, methods, and techniques for designing and implementing promotional and informational literature, displays, and resource materials.
• Considerable knowledge of management, budgeting, and administrative support practices.
• Ability to plan, organize, coordinate, and implement programs through other professional personnel.
• Ability to be innovative and creative in developing public health education activities.
• Ability to work effectively with a variety of people on all educational levels.
• Ability to develop and maintain effective working relationships with community agencies, private organizations, public sector groups, news media, and the general public.
• Ability to write and speak effectively and to stimulate interest and cooperation among various groups in implementing a public health education program.

Typical Qualifications

Minimum Qualifications

Master's degree in public health education or in public health with a major in public health education and two years of experience in health education work; or graduation from a four-year college or university with a major in health education and four years experience in public health education; preferably with one year supervisory experience.

- Bilingual

Driving is Essential

Driving is an essential requirement of this position whether driving a County owned or personal vehicle to conduct county business.  Motor Vehicle Reports may be verified for valid driver's license and that the driving record is compatible with the county's driving criteria.
If a personal vehicle is operated for county business proper insurance is maintained as per Guilford County's vehicle use policy.
