Job Description

Salary Range: $48.71 - $57.99

 

Estimated Hours: Saturday, Sunday and Monday; however, flexibility may be required to assist with covering vacations or to support patient flow needs.

 

Position Summary:

Reporting to the Manager of Medicine Programs, the CMH@Home Patient Navigator is responsible for collaborating with patients and their families to develop high-quality, timely, and cost-effective individual service plans based on patient needs. Utilizing a multidisciplinary approach, the goal is to achieve optimal health outcomes.

 

The primary purpose of this role is to assist patients in reaching their highest level of functioning and independence, in alignment with their values, priorities, capacities, and preferences for care. The Patient Navigator will determine eligibility for services in accordance with relevant policies and procedures and care pathways.

 

The Patient Navigator will provide comprehensive clinical services to patients and their families. This role includes crisis intervention, emotional support, consultation, counseling, completing referrals, and connecting patients to appropriate community services. The Patient Navigator will lead efforts to educate and counsel patients and families while facilitating referrals to support services, all aimed at addressing patients’ goals.

 

The Patient Navigator will liaise between the hospital care team including physicians, primary care and community support services partners including Ontario Health atHome.

 

Qualifications:

 

  • Bachelor’s Degree in a Health-Related Field Required; Master’s Degree Preferred.  
  • Regulated Health Professional with a current and in good standing registration with a regulatory college (ie RN, OT, PT, SW, SLP, RD)  
  • Acute care experience of 3-5 years preferred
  • Demonstrated knowledge of relevant legislation, including the Health Care Consent Act and Public Hospitals Act

  • Demonstrated knowledge of policy and procedure development and staff education implementation.  
  • Demonstrated experience collaborating with interdisciplinary teams in an acute care environment.
  • Excellent interpersonal and communication skills, with the ability to lead interdisciplinary discussions  
  • Ability to function effectively in a fast-paced, high-pressure environment.  
  • Strong problem-solving, decision-making and conflict resolution skills.  
  • Highly organized and detail-orientated with excellent time management and prioritization skills  
  • Ability to work both independently and collaboratively within a team  
  • Flexibility and adaptability to the changing needs of an acute care setting  
  • Demonstrates supportive emotional care to the patient and their family/significant others from all diverse backgrounds  
  • Demonstrates emotional intelligence to navigate high-stress situations with empathy, self-awareness, and professionalism, ensuring compassionate care for patients and respectful collaboration with colleagues.  
  • Ability to recognize and manage emotions—both personal and in others— Demonstrates trauma informed care to support a positive patient-centered care environment.  

Additional Information:   

  • This is a non-union position, with no guaranteed fixed hours. Flexibility is required to meet operational needs.   
  • Regular attendance and commitment to professional development is required  
  • May be required to attend off-site meetings with key stakeholders or participate in quality initiatives beyond program level.
