A Clinical Support Team Administrator is required to support the SMI Suffolk Physical Health Team, a team of health professionals focused on:
· A reduction in physical health illness in those service users across Suffolk both in primary and secondary care, with severe mental illness (SMI), approx. 3,800.
· An increased level of physical health screening for service users with complex health needs, in particular, the Annual SMI Physical Health check.
· Ensuring that the physical health needs of the service users are met through sharing healthcare information with the patients' primary care team and other stakeholders.
• Promoting service user self-management, self-care, recovery and the maintenance of independence.
• Improving communication between service users, professionals and carers.
• Ensuring a high level of service user and professional satisfaction
• Developing and maintaining successful, collaborative relationships with both primary and secondary care services.

To provide administrative support to the team in order that they can provide an efficient service to all customers.
