Position Summary:  

 Responsible for providing direct and comprehensive administrative support to the hospice IDG clinical team, including medical records management, scheduling, quality assurance activities, and IDG documentation assistance.

 

Hours / Schedule: Full Time; (40) hours / week. 

 

Minimum Qualifications:   

  • EDUCATION:
    • Diploma or degree in a healthcare-related field from an accredited college or university, or the international equivalent.
    • Licensed Practical Nurse (LPN) designation is preferred but not required.
  • EXPERIENCE:
    • Minimum of one (1) year of experience in clinical records management, preferably in hospice care operations.
    • Knowledge of information systems required.
  • CERTIFICATION/LICENSURE(S): o Licensed Practical Nurse (LPN) preferred.
    • Accredited Records Technician Certification preferred.

 

Key Responsibilities:   

  • Oversee daily clinical records functions, including the development and implementation of clinical records policies.
  • Ensure clinical record systems comply with state, federal, and ACHC regulations.
  • Maintain a thorough understanding of state, federal, and ACHC documentation regulations, serving as a resource for organization personnel.
  • Safeguard clinical records by implementing control procedures for both open and closed records.
  • Maintain patient and organizational confidentiality through effective control measures and direct supervision of clinical records.
  • Provide authorized users with copies of clinical records in accordance with policy.
  • Conduct administrative record audits upon patient discharge and submit records to the Clinical Manager for clinical review.
  • Track aged, unsigned physician orders and send second and third requests to overdue physicians.
  • Follow up with seriously overdue physicians through calls or in-person visits.
  • Supervise the use of the clinical records information system, maintaining expertise in system functions, upgrades, and enhancements.
  • Recruit, train, and manage clinical records team members.
  • Ensure accurate and timely filing and retrieval of clinical records by overseeing the clinical records team.
  • Provide direct support and assistance for scheduling activities as assigned.
  • Collaborate with clinical, administrative, and leadership teams to coordinate and communicate scheduling assignments that align with patient and family needs as outlined in the Plan of Care and IDG.
  • Perform other clinical support duties as assigned by management to support department goals and initiatives
