About the Job:
As a Clinical Support Coordinator, you will be responsible for supporting clinical operations related to the provision of comprehensive medication management services to patients in an ambulatory care setting. Your knowledge of medications, social determinants of health, and the healthcare system will play a critical role in optimizing operations. You will support a variety of clinical operations and quality measures related to medication management. The Clinical Support Coordinator is a highly visible customer service and patient-focused role which works directly with pharmacists, patients, caregivers, physicians, nurses, and other allied health personnel in a dynamic and professional environment to provide the highest level of quality healthcare to patients.
What you will do
- Obtain discharge summaries and other clinical documentation from health system partners.
- Conduct comprehensive interviews with patients to obtain accurate and up-to-date medication histories and allergy lists.
- Inform patients of medication management services they are eligible for and schedule appointments for them to meet with a clinical pharmacist.
- Assist with prior authorization requests.
- Facilitate patient enrollment into prescription savings programs.
- Send and follow-up on pharmacist-initiated clinical recommendations to prescribers and document actions taken.
- Provide outstanding customer service to patients, providers, and pharmacists.
- Assist with closing clinical gaps to improve CMS Star ratings and HEDIS measures.
- Participate in quality improvement initiatives to enhance the delivery of comprehensive medication management services.
- Champion diversity, equity, and inclusion efforts, fostering an equitable workplace.

What you will need
- Medical Assistant, preferred certified Medical Assistant (CMA) or
- Active Pharmacy Technician licensure in your state of residence in good standing with certification as a Pharmacy Technician from CPhT, PTCB, PTCE, ExCPT PTAC, or state- specific certification or
- Extensive customer service experience in healthcare settings
- Minimum 2 years clinical experience preferred.

- Excellent communication (verbal and written) and interpersonal skills. Must exercise proper phone etiquette.

- Commitment to patient-centered care and continuous quality improvement.
- Exceptional organizational skills with the ability to effectively prioritize and complete tasks in a timely manner.
- Strong knowledge of medications, including proper name pronunciation, commercially available strengths, and common indications is highly preferred
