There is currently one permanent 1 WTE Clinical Specialist Occupational Therapist in our inpatient Stroke Rehabilitation Unit.

A panel may be formed as a result of this campaign for St. Columcille’s Hospital from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.

St. Columcille’s Hospital is located on the border of County Wicklow. It is a model 2 hospital and is aligning itself to the Sláintecare vision. It has 117 inpatient beds and provides a range of services to a diverse population covering South County Dublin and County Wicklow. St. Columcille’s Hospital is committed to providing a quality, patient focused service in a way that meets all patient needs in an equitable, efficient and safe manner. We acknowledge the contribution of each member of staff and aim to encourage and support them in their on-going professional development. Services include a 7/7 Local Injury Unit, 7/7 Medical Assessment Unit, 113 medical inpatient beds incorporating Stroke & Ortho Rehab Units, Endoscopy, a 5/7 Day Surgery Unit, an Outpatient Department, Radiology and various diagnostic services for local GPs. St. Columcille’s Hospital acts as a hub and spoke model for integrated care. Located in St. Columcille’s Hospital is the National Centre for all obesity management and the National Gender Service.

The Occupational Therapy Department in St. Columcille’s Hospital provides a dynamic and holistic Occupational Therapy service to in patients and out patients attending St. Columcille’s Hospital. With strong relationships with our community partners and our multidisciplinary team colleagues we strive to deliver quality and meaningful interventions in an efficient manner.

Occupational Therapists in St. Columcille’s work to promote and enable independence in activities of daily living and to enable their patients to function at home as independently and safely as possible following a hospital admission, or medical event.

Therapists have access to rehabilitation space in the Rehabilitation and Activities suite where we can provide individual assessments / interventions and group interventions.

In patient services include Stroke Rehabilitation, Ortho-geraitric rehabilitation, Care of the Elderly, General medicine.

Out- patients are assessed and treated in the Day hospital, in the Occupational Therapy Department, in Cardiac Rehabilitation and in the Memory Technology & Resource Room.

The Frailty Intervention Team commenced in the Medical assessment Unit in November 2019.

The Occupational Therapy service actively supports student education and participates in the student training scheme with other Health & Social Care Professionals and with Irish Universities. Students can be on site for up to 9 weeks at a time. All students are allocated a named supervisor and engage in formal supervision weekly.

The Stroke Rehabilitation service accepts transfers of stroke patients from St. Vincent’s University Hospital for inpatient stroke Rehabilitation. The Stroke unit consists of 8 inpatient beds. SCH is currently an auxiliary service working alongside and as part of neuro-rehabilitation strategy. SCH works on collecting and collating data in line with the neuro-rehabilitation strategy & works closely with the identified specialist rehabilitation sites.

SCH has established links with the local Community Neurorehabilitation Team (CNRT) and is represented on the Local Implementation Group for same. This ensures easy transition to community services, enhanced communication and promotion of integrated care.

Reporting Relationship

The post holder will report to the Occupational Therapist Manager or Designated Officer.

Purpose of the Post

The Clinical Specialist Occupational Therapist will provide the advanced clinical knowledge and expertise to design and implement a high quality service. He / She will be responsible for the effective and efficient delivery of an evidenced based, patient focused, quality infused Occupational Therapy Stroke Service.

· All eligible candidates must be qualified 5 years and have 4 years consecutive or equivalent work experience in stroke care.

· To apply advanced Occupational Therapy (OT) knowledge, skills and experience to stroke survivors.

· The successful candidate will possess skills and expertise in the area greater than that expected of a senior grade. This will include research , service development work within designated area

· The development and maintenance of high standard of clinical practice within this area through clinical evaluation, completing audit and evidence based practice

· Serves as a leader in service improvement and development.

· Develops and implements clinical evidence based practice, research and audit in the occupational therapy stroke service,

· Provides expert guidance/consultation to other occupational therapists, students and health care professionals via education, supervision and training to ensure they have the required skills & knowledge to deliver evidenced based, safe and effective practice

Principal Duties and Responsibilities

Clinical Practice

The Occupational Therapist, Clinical Specialist (Stroke) will:

· Be responsible for assessment, diagnosis, planning, implementation and evaluation of treatment/intervention programmes for patients presenting in stroke care according to evidence base best practice and/or professional standards

· Be able to make highly specialist clinical decisions following assessment of but not limited to complex cases for patients within the clinical area.

· Assess and effectively manage own and OT team treatment towards the associated effects of patients’ within clinical area.

· Demonstrate advanced knowledge of Occupational Therapy evidence based practice in stroke care.

· Be responsible for a clinical caseload and support and delegate appropriately to Occupational Therapy staff working within their area.

· Ensure a high standard of Occupational therapy assessment, treatment and management is provided for patients under his/her care and ensure that professional standards of practice are adhered to.

· Promote changes in work practices, procedures and /or techniques having regard to the development of best practice and advanced practice in Occupational Therapy.

· Ensure the privacy and dignity of the patient is respected at all times.

· Promote changes in work practices, procedures, techniques having regard to the development of best practice and advanced practice in Occupational Therapy.

· Communicate with other clinical specialists throughout the country and internationally to further develop clinical excellence and research. Clinical Specialist Occupational Therapist

· Act as resource in his/her specialist clinical area to Occupational Therapists and other health care professionals regarding the management of complex/problematic cases.

· Advise own clinical team on the complex disease and symptom management for patients’ within the stroke speciality.

· Serve on and advise such committees that may be set up relevant to this area of clinical specialty.

· Be able to make highly specialist clinical decisions following assessment of but not limited to complex cases for patients within the clinical area.

· Work within the multidisciplinary team ethos and liaise with staff to ensure that effective communication takes place at all times.

· Keep abreast of research and practice developments in relevant clinical areas by attending seminars, conferences and post graduate courses.

· Serve on and advise such committees that may be set up relevant to this area of clinical specialty.

· Supervision of senior and staff grade Occupational Therapist within the OT service.

· Undertake other appropriate responsibilities consistent with the nature of the post.

Working Practice / Leadership

· Represent the occupational therapy profession and promote the profile of Occupational Therapy both in SCH and within stroke and at a national level.

· To work collaboratively with the OT manager and OT staff in providing leadership that will positively influence quality improvements within the OT service and respective clinical area at an organisational , regional and national level

· Interpret standards of practice in relation to respective clinical area, in addition to professional codes of conduct, professional and organisational policies ensuring these are incorporated into Occupational Therapy practice.

· Recognize when it is appropriate to refer decisions to a higher level of authority and include colleagues in the decision making process as appropriate

· Understand the principles of the operational management of their respective areas and be responsible for the day to day management of this clinical speciality area

· Provide leadership and assist in the setting and monitoring of clinical standards, policies and procedures, evaluation of clinical practice, quality control and clinical audit.

· Build and maintain good relationships with other services in the region and with agencies, both statutory and voluntary.

· Undertake other responsibilities as agreed from time to time with the Occupational Therapy Manager or other such persons to whom he/she may be accountable to.

· Be responsible for the maintenance of OT clinical equipment as appropriate to clinical area

· Keep up to date with change and developments within the Irish Health Service and look to review clinical area as appropriate in respect of these changes

· Demonstrate advanced knowledge of care pathways appropriate to clients availing of the service and proactively work with colleagues to design and implement processes to improve flow for clients.

· Design and implement processes and procedures to ensure the highest standards of practice are met in the occupational therapy service by establishing governance systems.

· Foster and lead a culture and practice of evaluating service outcomes appropriate to the specialist area and continually audit and evaluate the service with a view to maximising effectiveness, efficiency and quality.

· Collaborate and effectively communicate with the Occupational Therapy Manager(s) and multidisciplinary team regarding referral pathways to the specialist area and patient management to ensure a safe, effective and efficient service.

· Promote changes in work practices, procedures, techniques or technology having regard to the development of best practice and advanced practice in Occupational Therapy.

· Be aware of resource management and contribute to business planning and continuous quality improvement related to the Occupational Therapy Department and the specialist area.

· Assist in the setting and monitoring of clinical standards, policies and procedures, evaluation of clinical practice, quality control and clinical audit.

· Work collaboratively with the MDT to identify, plan and implement interdisciplinary practices.

Communication & Documentation

The Occupational Therapist, Clinical Specialist (Stroke) will:

· Maintain comprehensive and accurate patient and activity records in line with national documentation standards.

· Compile and prepare reports/statistics as requested.

· Compile reports of a legal nature where necessary, based on the documented Occupational Therapeutic care of the patient.

· Comply with advances in technology (patient e-records etc.).

· Communicate oral and written information in a clear concise and well-structured manner appropriate to the target audience.

· Build and maintain effective relationships with colleagues at clinical and management levels.

The Occupational Therapist, Clinical Specialist (Stroke) will:

· Maintain knowledge of and critically analyse current literature and available evidence relevant to the clinical area and integrate same into practice.

· Monitor and evaluate effectiveness of interventions within the service and modify accordingly where new evidence emerges.

· Develops and continually revise clinical guidelines for own area of practice in accordance with evidence from National, International and professional bodies guidelines.

· Puts in place evaluating mechanisms appropriate to the specialist area and demonstrate practices of continual audit and evaluation of the service with a view to maximising effectiveness, efficiency and quality.

· To demonstrate advanced level knowledge of evidenced based practice in clinical specialism through the process of clinical reasoning and decision making allowing knowledge to be applied in the complex/different situations

Act as Clinical Resource

The Occupational Therapist, Clinical Specialist (Stroke) will:

· Represent the Occupational Therapy Profession within their own area and across multiple service areas within the HSE and all relevant stakeholders.

· Promote and instigate initiatives that will ensure professional practice and advancement of standards

· Act in an advanced clinical advisory role to OT colleagues.

· Act as an expert occupational therapy clinical resource for the stroke/neurology MDT.

· Identify the need for appropriate onward consultation.

Health and Safety

The Occupational Therapist, Clinical Specialist (Stroke) will:

· Draft new and update existing policies and procedures relevant to their role to ensure a safe, effective, efficient service will be delivered.

· Be familiar with and ensure implementation of HSE and department policies and procedures, also ensuring compliance with Health and Safety legislation, Occupational Therapy Safety Statement etc.

· Collaborate with the Occupational Therapy Manager and deputy Occupational Therapy managers by contributing to the preparation of risk assessments and safety statement for the Occupational Therapy Department.

· Document all accidents, incidents and potential hazards immediately and report them to the Occupational Therapy Manager.

· Advise the Occupational Therapy Manager regarding any danger/deficiency in the specialist area.

· Report any emerging risk issues in a timely manner to the Occupational Therapist Manager and other relevant senior staff within their clinical team and OT service generally.

· Attend fire lectures, must observe fire orders and assist in maintaining the fire register.

· Participate as required in Occupational Health and Safety training.

· Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc. and comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role.

Terms and Conditions of Employment

Tenure

The current vacancy available is permanent and whole time.

The post is pensionable. A panel may be created from which permanent and specified purpose vacancies of full or part time duration may be filled. The tenure of these posts will be indicated at “expression of interest” stage.

Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointments) Act 2004 and Public Service Management (Recruitment and Appointments) Amendment Act 2013.

Working Week

The standard working week applying to the post is to be confirmed at Job Offer stage.

HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016).

Annual Leave

The annual leave associated with the post will be confirmed at Contracting stage.

Superannuation

This is a pensionable position with the HSE. The successful candidate will upon appointment become a member of the appropriate pension scheme. Pension scheme membership will be notified within the contract of employment. Members of pre-existing pension schemes who transferred to the HSE on the 01st January 2005 pursuant to Section 60 of the Health Act 2004 are entitled to superannuation benefit terms under the HSE Scheme which are no less favourable to those which they were entitled to at 31st December 2004

Age

The Public Service Superannuation (Age of Retirement) Act, 2018* set 70 years as the compulsory retirement age for public servants.

* Public Servants not affected by this legislation:

Public servants joining the public service, or re-joining the public service with a 26 week break in service, between 1 April 2004 and 31 December 2012 (new entrants) have no compulsory retirement age.

Public servants, joining the public service or re-joining the public service after a 26 week break, after 1 January 2013 are members of the Single Pension Scheme and have a compulsory retirement age of 70.

Probation

Every appointment of a person who is not already a permanent officer of the Health Service Executive or of a Local Authority shall be subject to a probationary period of 12 months as stipulated in the Department of Health Circular No.10/71.

Protection for Persons Reporting Child Abuse Act 1998

As this post is one of those designated under the Protection for Persons Reporting Child Abuse Act 1998, appointment to this post appoints one as a designated officer in accordance with Section 2 of the Act. Mandated Person Children First Act 2015

Health & Safety

It is the responsibility of line managers to ensure that the management of safety, health and welfare is successfully integrated into all activities undertaken within their area of responsibility, so far as is reasonably practicable. · Ensuring that all incidents occurring within the relevant department/service are appropriately managed and investigated in accordance with HSE procedures.

· Seeking advice from health and safety professionals through the National Health and Safety Function Helpdesk as appropriate.

· Reviewing the health and safety performance of the ward/department/service and staff through, respectively, local audit and performance achievement meetings for example.
