A Clinical Liaison Marketer for home health serves as the primary bridge between acute care facilities (like hospitals and skilled nursing facilities) and the home care agency. This role combines clinical expertise with relationship management to ensure that patients transitioning from a hospital to home-based care do so safely and without interruption in their treatment. Your key responsibilities would include:

• Patient Assessments: Conducting clinical evaluations of referred patients to determine if they meet the specific eligibility criteria for home health or hospice services.
• Care Coordination: Collaborating with hospital case managers, discharge planners, and physicians to facilitate a seamless "handoff".
• Relationship Building: Maintaining professional partnerships with referral sources (doctors and social workers) to ensure the agency remains a top choice for patient placement.
• Marketing & Growth: Tracking referral activity and implementing outreach strategies to help grow the agency's patient census.

Qualifications & Skills

• Licensure: Licensed Practical Nurse (LPN)
• Experience: At least 1 year of experience in home health.
• Essential Skills: Strong communication, empathy for end-of-life care, organizational ability

Schedule

• This full‑time position follows a Monday-Friday schedule, 8:00 AM to 5:00 PM.
